Form 990, Part VI, Section B, line 12c At each board meeting, the Board of Directors obtains information regarding conflicts and those abstaining for other reasons. This is noted and placed in the minutes which in turn become part of the Organization's public records. Form 990, Part VI, Section B, line 15a Comparability data is provided to the Board of Directors prior to approval of a bonus or raise. The Board of Directors votes on the raise or bonus. Form 990, Part VI, Section C, line 19 The Organization's governing documents, conflict of interest policy, and finacial statements can be requested at any time at the Organization's office. Form 990, Part XI, Line 2c Responsibility for Audit Oversight The Organization competitively procures auditing services by advertising requests for proposals from qualified accounting firms. A team of field experts rate each component of each respondent's proposal. Scores are totaled and the results are brought to the Board of Directors, who then approves which company contract negotiations will begin with. Annually, the audit report and the Form 990 are presented and approved by the audit committee, finance committee, executive committee, and the Board of Directors.
