Ammons Procedure for Monitoring Grants in the U.S.: Part I, Line 2: Schedule I, Part I, Line 2: (1)Organization has a grant award letter that is required to be signed by the head of the grantee Organization receiving the funds with specific terms of the grant; (2)A grant award budget is mailed to each organization signed by the Chairperson and Executive Director, indicating the line items and outright funds available to them. A designated fiscal agent is to be identified by each organization. That individual is responsible for making sure that the funds are used as specified on the grant award budget and for preparing a financial report at the end of the project; (3)Staff authorizes payment of funds when the signed grant award letter and fiscal agent form are signed and returned to us; (4)Phone calls and emails with these organizations are documented and kept in their individual files; (5)Reporting forms to be completed by the Project Director and the Fiscal Agent of the organization are mailed to the organization and is also available on our website to be filled out at the end of the project. Organizations are given 90 days after the end of their projects to complete that report. If delinquent, staff follows through to receive the report. (6)An interim financial report is required for grants over $5,000 before further payments are approved; (7)Staff audits the report to make sure they comply with the terms of their grant as specified in the grant award budget (8)Organizations are required to maintain all records for a period of five years in case they are called upon for an audit. Form 990, Part VI, Section B, line 11 The Form 990 will be reviewed by the Executive Director and the finance chair prior to filing with the IRS. Form 990, Part VI, Section B, line 12c Council and staff members have the responsibility to report to the appropriate level of administration within the Council any material breach or failure of the Council's internal controls, investment function, auditing function, accounting systems, or governance policies of which they have actual knowledge or a reasonable and good faith belief. If a Council or staff member becomes aware, or has a good faith belief, that such a breach or failure has occurred, or may be about to occur, the Council or staff member should report his or her concern immediately to the appropriate Committee Chair, in the case of a Council member, or to the Executive Director, if a staff member. Examples of such material breaches or failures may include, but are not limited to, the following: Supplying false or misleading information in the Council's financial reports, including the tax return Misuse of Council Funds Providing false information to or withholding material information from the Council's auditors Violations of the conflict of interest policy Self-dealing, embezzlement, or fraud If the Council or staff member believes that normal channels of communication through a Committee Chair or the Executive Director are inadequate or inappropriate, the complaint should be reported with appropriate promptness to the Chair. Reports should normally be submitted in writing, but will be accepted if made orally. Reports should contain enough information to substantiate the concern and allow an appropriate investigation to begin. All reports will be received and acted upon in confidence to the maximum extent permitted by legal requirements and the need to gather facts, conduct an effective investigation, and take necessary corrective action. Following investigation, the Council will take any appropriate remedial and disciplinary action deemed justified by the circumstances.
