<Explanation>Schedule I, Part I, Line 2: Wood County Hospital has a scholarship program to provide financial assistance to individuals preparing to pursue a career in health care. To apply for a scholarship, the candidate must live in an area served by Wood County Hospital, be a high school graduate or equivalent with GED, and be accepted at his or her chosen school. The candidate must complete an application, which details the candidates reasons for choosing health care. Applications are reviewed by a committee consisting of the Staff Development Coordinator, the Vice President of Employee Services, and the Administrative Assistant. The area of specialization in health care selected, community need for more professionals in a particular field, the applicants community and volunteer participation, and financial need are instrumental in the committees decision-making process. Each applicant selected will receive a scholarship in an amount ranging from $200 - $500. The scholarships will not exceed $1,000. The hospital budgets approximately $9,000 annually for this program. During the 2010 Fiscal Year, we awarded 7 scholarships totaling $3,500.</Explanation>

Four area schools participated this year and 238 students were seen. Individuals are provided a special container to collect used needles and other sharp medical waste. The full containers are returned to the hospital for proper disposal and a new container is dispensed. The program is geared toward individuals only; businesses are not eligible to participate. There are now more than 1382 residents involved, with 147 new participants this year. There were 1054 containers returned this year for safe destruction. Form 990, Part III, continued Wheeled Meals are prepared for community residents who are unable to prepare meals for themselves. This year, 35,772 meals were provided at a subsidized fee. CPR training courses are provided at a nominal cost. Local organizations including schools, government agencies, and private businesses are represented in the classes. (ie. Children's Resource Center, Eastwood Schools, BGSU, Pioneer Seed, Oak Grove Extended Care and the City of Haskins are some of the organizations that we provide service to). Approximately fifteen area organizations participate each year. The Lifeline Program is an emergency response system for people who live alone or need an emergency response system. One of the many services offered by Wood County Hospital there is a $20 monthly fee and a $20 one time installation fee. If you are not a Wood County Hospital user the costs are $30 per month and $30 one time installation fee. If the patient is unable to pay service will be provided. Currently we have one subscriber not paying for the service. To date 325 subscribers in the greater Wood County area are on the Lifeline Program. All calls come into the Hospital and over 7,920 calls were handled over the past year. The Comprehensive Sports Network, administered by the Physical Therapy Department, holds a free sports medicine clinic in May. Its primary objective is to provide timely evaluation of injuries resulting from scholastic sports. Although it emphasizes student athletes, the clinic accepts anyone with a sports related injury. The Network provides athletic trainers to three area high schools.</Explanation>
