On-site visits include reviews of files and compared with information entered in the shared database, procedural reviews and discussions with management. Program personnel review activity of the grant recipients on an on-going basis by reviewing activity in the shared database and review of various financial and program reports required of the grantees. Grants to individuals require authorization by case managers and supporting documentation prior to payment being made. There are a large amount of grants to individuals that are to pay for tuition and books, these are normally paid directly to the institution and the organization is listed on account. Form 990, Part VI, Section B, line 11 The prepared Form 990 is reviewed by the organization's executive director and finance manager before the return is filed with the IRS. Form 990, Part VI, Section B, line 12c All board members, contractors, and employees are required to sign disclosure statement annually. Form 990, Part VI, Section B, line 15 The Executive Committee annually conducts an evaluation of the Executive Director and approves compensation as part of the annual budget process which requires a vote of the Executive Committee. The Board has utilized salary survey information compiled by the State of Wisconsin specially for Workforce Development Board key employees. The Executive Director follows the same process for the other executive positions within the organization -same salary survey is utilized. A vote by the Executive Committee is required for all compensation included within the corporate budget. Governing documents are included in the Workforce Investment Act strategic plan that is made available for a 30 day public review period prior to submission semi-annually.
