Each responsible person must annually complete a disclosure form identifying any relationships, positions, or circumstances that could contribute to a potential conflict of interest. The disclosure form must be submitted to the Executive Committee. The Executive Committee is responsible for reviewing potential conflicts of interest and handling what it determines to be actual conflicts of interest in the manner described in the policy under "Reviewing and Managing Conflicts of Interest". If the Executive Committee determines that an actual conflict of interest exists with respect to any other responsible person, the Executive Committee will determine the appropriate response as described in "conflicting interest transactions" below. The Executive Committee will report to the Board at its next regular meeting the results of any review and the action taken.
