63 Procedure for Monitoring Use of Grant Funds Inside U.S. Schedule I, Part I, Line 2 The Providence Plan follows HHS guidelines and submits our procedures for their approval. We require an application from each grantee, submitted by a set deadline. We use a group of community leaders to choose grantees, based on a uniform set of criteria, equally applied. Staff provides support to the process but does not choose the grantees. Once chosen, staff and grantees work together to create a workplan laying out the specific activities the organizations will carry out with the money. We both sign the workplan, and a Memorandum of Agreement, which lays out the proper use of the funds. Grantees must provide periodic reports on their spending. Grantees must alert staff to any change in their workplan, and staff must create a new workplan for any change that affects more than $1000 of their grant. Besides receiving grantee reports, staff hold regular calls with grantees, to monitor the appropriateness of their spending. A copy of the Form 990 as it is ultimately filed with the IRS is provided to all of the board members prior to filing the 990. Conflict of Interest Policy Monitoring & Enforcement Form 990, Part VI, Line 12c Annually employees and members of the board of directors sign a conflict of interest statement attesting to any existing conflicts, or affirming no conflicts exist. Board members are required to recuse themselves from any vote on transactions which may pose a conflict of interest or an appearance of a conflict of interest. The Board of Directors shall be divided into 5 classes. Each class will have one member each from the list of the appointees of the governor and mayor.
