We accomplish this by providing in-home services to help homebound older people live in their own homes as long as possible; helping active older people stay fit, healthy, and involved; and helping family caregivers navigate the maze of available services and care for aging family members. Our vision is a community of people who celebrate living and are aging well. The six-county area served by CareLink includes Faulkner, Lonoke, Monroe, Prairie, Pulaski, and Saline. Clients receive assistance with the day-to-day tasks associated with housekeeping and personal grooming. Clients may also received nutritious meals-on-wheels, the companionship of another older volunteer, and the reassurance of personal safety provided by CareLink operators through regular phone contact or a handy alert monitoring system. Approximately 4,390 different individuals were served by the CareLink home-based program during the year ended June 30, 2011. 1591468 110355 Center-based Program: CareLink helps active older people stay fit, healthy and involved through fitness and wellness programs, senior centers and volunteer opportunities. At senior centers, people can enjoy a nutritious lunch, make new friends, renew old acquaintances, enjoy group activities and relieve the stress of being alone. A group transportation option provides shopping trips and other outings. Fitness and wellness programs are provided in senior centers and other locations. CareLink also offers a variety of volunteer opportunities. In fact, CareLink receives over $1 million a year in volunteer effort from dedicated volunteers.Approximately 3,388 different individuals were served by senior centers and the respite center during the year ended June 30, 2011. 1322926 95693 Outreach Program: CareLink helps older people and their caregivers determine what services are available. CareLink helps find the best way to pay for these services and either delivers them directly or makes referrals. Information and Assistance serves as a clearinghouse of useful resources on virtually every aging-related topic. Care coordinators help families match their needs with programs, services, and opportunities. Legal assistance and nursing home ombudsman programs provide additional support. A new service, family caregiver counseling, helps family caregivers navigate the maze of available services and care for aging family members.Approximately 9,639 different individuals were served by the CareLink care coordination, education, and advocacy program during the year that ended June 30, 2011. 1111254 1021330 Client-directed Services Program: CareLink operates the CareChoices program which allows individuals to direct their own care while remaining at home. Individuals in this program receive a flexible monthly budget based on a professional assessment of their needs. Using these resources, participants decide for themselves what mix of goods and services to purchase that may include home care workers (which may be family members or friends), home repairs/modifications, and medical transportation. The CareChoices program provides counseling, support services, and fiscal/employer agent services (bookkeeping, bill payment, and payroll/tax services) for all participants. Grantees submit monthly billings which are reviewed for allowable services. Annually CareLink staff perform service to billing audits and financial assessments of each grantee. Adjustments are made for unallowable charges. Grantees are required to submit annual audits. Management response is required from grantees to all findings and questioned costs.
