White RoadSan Jose, CA 95127MISSION AND PROGRAMSMissionThe mission of MOMENTUM for MENTAL HEALTH (MOMENTUM) is to help individuals achieve mental and emotional health, discover and reach their potential and fully participate in life. We believe all people with mental illness - our family members, neighbors, co-workers, and friends - have the right to receive effective treatment. United Way Silicon Valley awarded MOMENTUM a score of 96 out of 100 for its dedicated staff, clear understanding of needs, and treatment of individuals, while running a fiscally-responsible organization. CARF, a national accreditation agency gave MOMENTUM a three year accreditation which is the longest permitted.Each year MOMENTUM, the largest nonprofit provider of adult mental health services in the Silicon Valley, serves more than 3,000 youth, adults, and older adults with the most serious mental illnesses - Schizophrenia, Bipolar Disorder and Major Depression. These illnesses are treatable. Most people who participate in effective, timely treatment can recover and lead productive, satisfying lives. Continuing financial support from individuals, businesses, government and the community at large, however, is needed for MOMENTUM to continue to provide effective treatment programs.ProgramsMOMENTUM provides an extensive array of services designed to assist people with psychiatric diagnoses to work, enjoy family, recover from the illness and contribute to the community. Two of the most compelling examples of the ways in which MOMENTUM addresses critical community needs while providing clients services are its Employment Services and Intensive Services programs.Employment Services: The program's goal is to assist clients who have a serious mental illness, and an interest in returning to work, obtain and retain competitive employment. MOMENTUM provides employment preparation, job development, placement and job retention support, and consults with employers on disability-related issues.Intensive Services Programs: MOMENTUM has established two specialty outpatient programs designed to work with individuals who have serious mental illness, have not benefited from traditional mental health services and are historically frequent users of high-cost resources such as emergency rooms, acute inpatient hospitals, or jails. These programs are examples of MOMENTUM's commitment and ability to provide effective services to people with the greatest need while reducing the long-term price paid by them, their families and society when they do not receive effective, planned treatment .MOMENTUM offers a full range of clinically effective services that are designed and delivered with respect for the special needs of different cultures and ages. Services are provided in a several languages.Assessment and Customer Service: Centralized Service that evaluates the needs of those referred and admits to an MOMENTUM service or refers to another provider.Transition Age Youth Services: Outpatient service for adolescent and young adults who have a diagnosis of mental illness. The goal is to assist them continue with school or move successfully into the work force. Many services are also provided to their families.Crisis Residential Treatment: Crisis residential treatment proved therapeutic service in a 24-hour residential program as an alternative to hospital admission or continued hospital stay for individuals who need assistance in stabilizing acute psychiatric symptoms.Day Rehabilitation Services: These programs help individuals who have serious mental illness recover and resume daily activities. Services are intended for those who need help managing symptoms, assistance in maintaining and /or improving their life circumstances, as well as developing social and vocational skills. Peer Counseling Classes are also offered through this service.Adult Transitional Residential: The transitional residential program provides rehabilitation services in a non-institutional, residential setting where individuals are supported in their efforts to restore, maintain and apply interpersonal and independent living skills. Staff are available twenty-four (24) hours per day, seven (7) days per week (24/7). Combines services including Residential Care Facility, Day Rehabilitation and Outpatient The service provides 24/7 staffing, lodging, meals, medication assistance, life skills, development and links to community support for after care in addition to day rehabilitation and outpatient services.Medical Services: Psychiatric evaluations, diagnosis, treatment and medication services including medication education and referral for physical health treatment.Outpatient Services: Case management, therapy, psycho-education, counseling, treatment planning, rehabilitation services and coordination of care for adults and older adults with serious mental illness. Family education and support are also provided.ADDITIONAL COMMENTSImpact of Mental Illness:According to the National Mental health Association, more than fifty-four (54) million Americans - about one in five people - are affected by one or more mental disorders each year. Without effective treatment this can be devastating. Mental illness results in the second highest rate of disease burden in developed economies - even higher than cancer. Four of the ten leading causes of disability in the U.S. are mental disorders (Major Depression, Bipolar disorder, Schizophrenia and Obsessive-Compulsive Disorder).It is estimated that in Santa Clara County 95,000 of our neighbors, colleagues, employees, friends and family members have a mental illness. About 19,000 people are in need of mental health services and unable to access them. (Source: United Way Silicon Valley Trends Report).Funding:MOMENTUM's programs are funded primarily through contracts with county, state and federal partners, as well as private philanthropic contributions, including United Way Silicon Valley. MOMENTUM is seeking to increase contributions from individuals, small businesses, corporations and foundations in order to broaden its base of support and engage the entire community in its work on behalf of people with mental illness.Advocacy:MOMENTUM is a leading advocate for mental health education. The agency offers a "Rise Above Stigma" panel of consumers and families members that speaks to community groups, service club, the faith community and educational institutions.Innovations: Bipolar Disorder ProjectMOMENTUM is constantly seeking new and innovative ways to address critical needs in the community. Our goal is to "Bring Science into Service" by applying knowledge learned in academic settings to clinical practice in the community.MOMENTUM implemented specialized groups for people who have Bipolar Disorder after completing a research project funded by the Health Trust in collaboration with Pacific Graduate School of Psychology. Results of the Bipolar Project and the support programs developed as a result of it have helped people better manage their illness and thus reduce their need for mental health services and to move from being recipients of public benefits to becoming taxpayers.MOMENTUM has the staff needed to effectively serve our multicultural community:Delivered by a competent staff who speak over twenty-five (25) languages, MOMENTUM offers its multicultural clientele innovative services to facilitate their recovery from the impacts of mental illness.These services encourage hope and recovery by inviting each client to partner with MOMENTUM in developing their individualized treatment plan and by assisting him/her in recognizing and utilizing personal assets, skills and reach individual goals.
