The following three sections describe how input from persons with special knowledge or expertise in public health, state and local health departments, and leaders or representatives for underserved, minority, low income, populations or populations with chronic disease was accounted for during this CHNA process.1.0 Accounting for the Input of Persons with Special Knowledge or Expertise in Public HealthMeetings were held between representatives of the Community Benefits Department and persons with special knowledge of public health and the community from June to August 2013. Meetings were conducted in person or by phone. Persons with special knowledge of public health and the Houston Methodist community included an academic from The University of Texas School at Austin School of Biological Sciences, academics from the Institute for Health Policy at The University of Texas Health Science Center School Public Health, an academic from the Kinder Institute for Urban Research at Rice University, representatives from the Harris County Health Care Alliance, a representative from the Harris County Medical Society, representatives from the United Way and county affiliates, a representative from the Houston Methodist department of Care Management and Social Work, and a representative from Houston Methodist Hospital Operating Division. Through these meetings, recommendations were received from persons representing the broad interests of the community, requested and received recent datasets and publications, received consultation for methodologies used in this community health needs assessment, and identified potential health needs currently not being addressed in the Houston Methodist community.2.0 Organizational InputDuring the course of engaging the broad interests of the community, representatives from state, regional, and city health departments as well as organizations involved in health care were engaged. Meetings included representatives from the Department of State Health Services Center for Health Statistics, the Health Service Region 6/5 South office, Regional Health Partnership Plan Regions 2, and the Houston Department of Health and Human Services. During these meetings, recent datasets, publications and representatives' perspectives on community health needs were received. 3.0 Community Leaders and RepresentativesInput from community leaders and representatives of medically underserved populations, low income populations, minority populations, and populations with chronic disease needs was also collected through phone interviews. Participants were asked the following questions: 1. What community does your organization serve/represent? 2. What does a healthy community mean for your community? 3. What are current barriers to a healthy community in your community? 4. What are the top three health needs of the community you serve? 5. Are you experiencing any significant change in client population? 6. Is there a health need in your community that's not self-evident? Community leader input was included in the analysis of primary and secondary data in order to identify community needs and barriers, as well as establishing community priorities. Houston Methodist financial counselors actively seek all uninsured inpatients and outpatients with scheduled services for a personal visit. During these visits, the financial counselors will screen the patient for financial assistance and will provide the patient with the financial assistance application. The patient will then be provided a list of resources with contact information should the patient require further assistance in completing the application.Houston Methodist utilizes an eligibility partner to work closely with patients to qualify for State and Federal assistance programs (e.g., Medicaid, SCHIP, Crime Victims, etc.). This eligibility partner will meet with all uninsured inpatients to discuss State and Federal assistance programs; where the patient is not eligible for these programs, Houston Methodist's internal financial assistance program is introduced.Houston Methodist's case management unit works closely with the patient during patient discharge management to ensure the patient is introduced to all resources that may be needed post discharge (e.g., home health, skilled nursing, etc.). The case management unit works with the patient to qualify the patient and to gain access to these resources. Additionally, the case management unit works closely with the financial counselors when the patient is identified as having a need for financial assistance.Houston Methodist's patient accounting unit works closely with all uninsured patients post discharge during the billing and collections process. Houston Methodist's collections personnel will verbally screen patients for financial assistance during collection calls; where applicable, the patient will be sent a financial assistance application either in English or Spanish. Additionally, all billing statements provide the patient with a phone number for customer service to obtain payment options. Houston Methodist utilizes outside collection assistance, for both early out and bad debt collections. Houston Methodist Willowbrook Hospital is located in northwest Houston, which is approximately 18 miles from downtown Houston, Texas. Houston Methodist Willowbrook Hospital's primary services area is comprised of Harris County and Waller County. The Houston Methodist Willowbrook Hospital community spans 2,216 square miles and for 2014 contains an estimated population of 4,427,780 people. Harris County accounts for an estimated 4,386,666 (99 percent) of community residents and Waller County accounts for 41,114 (one percent) of community residents. The specific make-up of the service area includes Caucasian 32 percent, Hispanic 42 percent, African American 18 percent, Asian 6 percent and other 2 percent. Harris County has the highest average household income ($73,104). Waller County currently has the lowest average household income ($68,608). Examining the service area at the insurance level, the area consists of 77.9 percent insured and 22.1 percent uninsured.Houston Methodist West Hospital is located in the western part of Harris County, approximately 21 miles west of downtown Houston, Texas and opened on December 17, 2010. As of December 31, 2013, the hospital had 235 operating beds, 18 operating rooms and 846 affiliated physicians. Houston Methodist Sugar Land Hospital reported 13,563 inpatient adult admissions, 125,451 outpatient visits and 39,292 emergency room visits for 2013. Community Hospitals Houston Methodist Willowbrook HospitalOpened in 2000, Methodist Willowbrook Hospital offers care and technology to the northwest Houston community. As of December 31, 2013, the hospital had 241 operating beds, 14 operating rooms and 756 affiliated physicians. The hospital has recently expanded facilities and services to meet the needs of most patients requiring tertiary care offering cardiovascular surgery, neurological care, perinatal and pediatric care, as well as specialized services at its cancer and breast centers. Houston Methodist Willowbrook Hospital reported 14,471 inpatient adult admissions, 79,271 outpatient visits and 56,527 emergency room visits for 2013.Community Hospitals - Houston Methodist West HospitalHouston Methodist West Hospital opened its doors to the west Houston and Katy communities in December 2010. Located at I-10 and Barker Cypress, it offers the community specialty medical services such as a state-of-the-art breast imaging center, cancer center, cardiology and cardiac surgery, neurology and neurosurgery, orthopedics, sports medicine, birthing center, neonatal ICU and robotic surgery. As of Dec. 31, 2013, the hospital had 146 operating beds, 15 operating rooms and 659 affiliated physicians. Patients served in the hospital are from many different segments of these communities with a diverse mix of financial resources as well as cultural and socio-economic backgrounds. The hospital also made donations to the Alzheimer's Association, American Cancer Society, American Diabetes Association, American Heart Association, Baytown Chamber of Commerce, Baytown United Way, Hispanic Chamber of Commerce, West Chambers County and North Channel Chambers of Commerce, Sterling High School Partners in Education Program, Baytown Interfaith Ministries and other non-profit organizations. In 2013 Houston Methodist San Jacinto Hospital, through the Houston Methodist system's external charity care program, provided a total of $8.3 million for services to the medically needy through external organizations that are already meeting the health care needs that Houston Methodist has identified as priorities. To help improve health care access to the community, Houston Methodist San Jacinto Hospital provided $16.7 million in financial assistance based on the IRS definition of cost. The hospital provides financial resource counseling through all departments including the Emergency Department, to assist patients with financial assistance including screening for government assistance and charity qualification. Full-time employees are dedicated to providing this counseling, and one dedicated employee provides ongoing follow-up on charity applications. Improving Community Health and Well-BeingIn addition to Houston Methodist's contributions to community health through charity care and community benefits, each Houston Methodist hospital actively develops awareness initiatives, sponsorships and programs that promote health and wellness, prevention and early detection. Houston Methodist staff and volunteers can be found at health screenings, exercise and nutrition promotions, stress management clinics and other health promotion activities throughout the year. For example, Houston Methodist Sugar Land Hospital, Houston Methodist Willowbrook Hospital, Houston Methodist West Hospital and Houston Methodist San Jacinto Hospital participated in Houston Methodist's seventh annual Stride4Stroke 5K Walk/Run, which raised more than $550,000 in 2013. The walk/run is part of Houston Methodist's campaign aimed at increasing stroke awareness and education in the community. More than 2,800 employees and community members participated in this drive to raise public awareness and funds for an event that clearly has an impact on the community.Houston Methodist also reaches out to the community through health screenings, blood drives, health and safety tours and programs for children and teens. The Men's Health Expo is one of the community outreach events - those who attend received free health screenings, including PSA, cholesterol and glucose, body fat, blood pressure, flexibility tests and massages. This is one of more than 50 outreach events held with our community partners throughout the year. In addition to participating in Houston Methodist events, Houston Methodist Sugar Land Hospital, Houston Methodist Willowbrook Hospital, Houston Methodist West Hospital and Houston Methodist San Jacinto Hospital also participate in community supported events in their respective service areas as delineated below.Each community hospital supports their respective communities through sponsorships of local organizations' activities, physician speaking engagements, free seminars on health and medical topics and health screenings. Community Support Houston Methodist Sugar Land HospitalHouston Methodist Sugar Land Hospital collaborated with several agencies in 2013 to promote health awareness and offer screening programs to the Fort Bend and surrounding communities. The following screenings and health knowledge seminars were conducted: Vein Seminar, Weight Management Seminar, Colorectal Cancer Seminar & Screening Kit, Flu Shot Event, Migraine Seminar, back and neck pain seminar, Stroke Seminar, Cancer Survivor's Celebration & Luncheon, Sports Physicals event, Joint Pain Seminar, Breast Cancer Survivors Support Group, Grief Support Group, Spirituality Seminar and Caregiver Seminar.Houston Methodist Sugar Land Hospital conducted several educational programs for the community. Approximately 11,000 participants attended these programs/seminars. We scheduled 132 mammograms while at the event. HMWB recorded 79,271 Outpatient visits, 14,471 Admissions and 56,527 Emergency Room visits for a total of 150,269 visits in 2013. 1c. HMW is located in the western part of Harris County, approximately 21 mile west of downtown Houston, Texas. On December 17, 2010, HMW opened its emergency center, operating rooms, ICU, surgery center and all ancillary support services. The hospital has opened additional specialty & outpatient areas such as a cancer center, birthing center, cath lab, breast center, cardiac imaging and physical therapy in addition to a cardiovascular program. During 2013 HMW recorded 85,910 visits which were comprised of 50,075 Outpatient patient visits, 27,497 Emergency Room visits and 8,338 Inpatient Adult Admissions. Currently, HMW has 161 licensed operating beds. The campus master plan includes expansion space to take the inpatient capacity to 400 beds, and to allow four professional buildings and two parking garages. 2. San Jacinto Methodist Hospital provides hospital services out of two locations in Baytown, Texas, which is approximately 30 miles east of downtown Houston, Texas. Two sites have adjacent professional office buildings that lease office space to physicians who practice within the hospitals and house hospital functions. As of December 31, 2013, Houston Methodist San Jacinto Hospital had 275 hospital beds in operation. Principal medical services provided include women's health, obstetrics, cancer center, urology, orthopedics, diabetes education, cardiovascular lab, rehabilitation, intensive care medical and surgical services and emergency care. A full service psychiatric program is included, as well as a skilled nursing facility. Houston Methodist San Jacinto Hospital operates the only hospital emergency room in Baytown and served its community in 2013 by providing 12,360 Inpatient Admissions, 50,585 Emergency Room visits, and 60,042 Outpatient visits for a total of 122,987 visits. 3. TMH Medical Office Buildings is operated to support The Methodist Hospital, doing business as Houston Methodist Hospital, Houston Methodist Willowbrook Hospital, Houston Methodist San Jacinto Hospital, Houston Methodist Sugar Land Hospital, and Houston Methodist West Hospital. The TMH Medical Office Buildings also operates office buildings connected, attached or proximal to these five hospitals. TMH Medical Office Buildings provides space for hospital departments and support services. In addition to these services, TMH Medical Office Buildings leases space to hospital employed physician practices in support of these hospitals. 4. Diagnostic Center Hospital of Texas (DCH) owns hospital assets that are leased under the terms of a long term lease to Houston Methodist Hospital (MMH). HMH in turn utilizes these assets, along with other substantial assets that it owns, to provide direct patient care. DCH also provides floating nurse staff to HMH. 5. The Methodist Hospital Research Institute, doing business as Houston Methodist Research Institute is a supporting organization of the Houston Methodist Hospital. It is a relatively new organization and is one of the cornerstones of the strategic vision for the future of the Houston Methodist Hospital, a top-ranked academic medical center hospital. Houston Methodist Hospital's reputation for excellence in patient care and its commitment to improving patient outcomes represents a significant motivation in the development of the Research Institute. Established in 2004, the Institute conducts groundbreaking translational and clinical research. Examples of some research projects conducted at the Institute are: Cardiovascular Regeneration Under the leadership of John P. Cooke, M.D., Ph.D., Chair, Department of Cardiovascular Sciences, the Houston Methodist Research Institute has created the Center for Cardiovascular Regeneration within the Department of Cardiovascular Sciences. The Center for Cardiovascular Regeneration aims to restore cardiovascular health by reversing cardiovascular aging and regenerating cardiovascular tissue. Cardiovascular regenerative therapies developed here may include small molecules, nucleic acid or protein-based agents, cell or tissue therapies alone or in combination with bioengineered matrices, nanotherapeutics, and medical devices. Recruitment of John Cooke, a world-renowned expert in fundamental and translational research in endothelial biology and vascular diseases, brought $2.5 million in grant dollars, four faculty and 5 postdoctoral fellows to the program and 41 publications and conference activities for the year. Atrial Fibrillation Atrial Fibrillation or AF is the most common sustained cardiac arrhythmia in adults and is a leading cause of stroke, disability, and increased mortality. Traditionally, doctors treat AF by using a metal-tipped instrument to ablate, or "burn," areas in the left upper chamber of the heart. This creates scar tissue, and when the treatment is successful, the scar tissue prevents AF. Catheter ablation however is less successful in persistent AF that lasts more than 7 days. Miguel Valderrabano M.D., Chief, Division of Cardiac Electrophysiology, Houston Methodist DeBakey Heart & Vascular Center, is now conducting a multicenter clinical study with the long-term objective of determining if alcohol, when infused into a vein in the heart and combined with standard treatment, can improve AF outcomes. This is the first federally funded clinical trial at the Institute with NIH funding of $3.5 million over 5 years. Transplant Immunology The goal of the transplant immunology program is to develop highly effective therapeutic protocols for supporting transplant survival without life-long immunosuppression and to eliminate complications from rejection of grafts in transplant patients. Xian C. Li, M.D., Ph.D., joined the Institute in 2013 as Scientific Director, Center for Immunobiology and Director, Transplant Immunology Program. 2013 also saw the addition of 4 other faculty members: Wenhao Chen, Ph.D., Roger Sciammas, Ph.D., Zhiqiang Zhang, Ph.D., and Xiang Xiao, Ph.D. to the program. With $1.5 million in total funding for 2013 (NIH grants of $877,000, Other grants of $147,000, and Clinical trials of $484,000), this program has produced 39 publications and conference activities for the year. The Houston Methodist Research Institute established the Cockrell Clinical Trials Centers to support clinical trials, especially the transition from preclinical to phase 1 & 2 studies. Kapil N. Bhalla, M.D., Phase I Program Director, was recruited to establish the Phase I unit of the Cockrell Centers. Cockrell Centers-Phase I unit initiated their first study on December 19, 2013. Currently there are 7 active and 8 upcoming trials at the Cockrell Center in the Texas Medical Center location, with ongoing expansion to other community areas. The Cockrell Center at the Sugar Land network hospital successfully opened its doors and is actively promoting clinical trials in the Sugar Land community. The West Houston site is supporting clinical trials and is also working towards opening a new office in 2014. Both facilities have part-time nursing staff to support the studies. The Pearland facility was newly formed in 2013 and work is currently underway to integrate the new facility, resources, and studies. Several staff members have been hired to support the centers. Jenny Chang, M.D., Director, Houston Methodist Cancer Center, was the Principal Investigator for the first clinical trial at the Cockrell Centers that studied 'Super Herceptin' therapy for breast cancer. Herceptin (Trastuzumab) is an approved adjuvant therapy for HER2+ early-stage breast cancer, metastatic breast cancer, and gastric cancer. It works by interfering with the HER2 receptor that promotes cell growth. In certain cancers, function of this receptor is amplified, leading to uncontrolled cell growth. Blocking its function with Herceptin therapy can be effective in combination with chemotherapy and/or radiation. Dr. Chang's team has developed 'super Herceptin' known as T-DM1. By combining Herceptin directly with docetaxel (standard chemotherapy) and/or pertuzumab, T-DMI is designed to attack the tumor cells directly and deliver the chemotherapy. Dr. Chang's trial was conducted in partnership with Genentech & ImmunoGen. In 2013, HMH was also activated as a site for the expanded access study. The Houston Methodist Research Institute has also made huge strides in the development of technology platforms that have applications across all service lines of the hospital. For two platforms, collaborations have already been established to evaluate heart, neurology, transplant, and cancer-related clinical applications. The two platforms are: -Microfluidic devices: Human blood is filled with protein biomarkers. Rapid assay systems can detect the presence or levels of these biomarkers in a blood sample - in many cases providing almost immediate diagnostic information. Most rapid assay systems, however, require pumps, power supplies, and fluid handling systems, limiting their portability and utility. Lidong Qin Ph.D. from the Department of Nanomedicine and his colleagues have developed a tiny, self-powered chip called the V-chip that uses the energy output from a chemical reaction to separate blood into plasma and cells, then rapidly determines the presence and levels of numerous diagnostic proteins in the plasma. The physician or lab technician only needs to place a tiny blood sample on the chip, then press a button on the surface. This device can be used to: o Isolate tumor-initiating cells o Detect biomarkers in serum, enabling fast accurate diagnosis at the bedside (published in Nature Communications) o Detect the drug susceptibility of TB (published in Nature Scientific Reports) Dr. Qin has received a $2.1 million grant from National Institute on Drug Abuse (NIDA) for application of the V-Chip to rapid point-of-care drug testing. He also received another $2.1 million from the National Cancer Institute to investigate whether this small, low-cost device can help assess a person's risk of developing a common form of liver cancer through blood tests. -High throughput assay for detecting tumor cells: Youli Zu, M.D., Ph.D., Medical Director, Hematopathology received the TechConnect World Summit & Innovation Showcase Award in May 2013 for developing a high throughput one-step assay for detecting circulating tumor cells in whole blood samples.
