More than 200 high-need clients, including some with families and some who are HIV, are assisted with acquiring and maintaining stable housing. 15026737 Health Insurance Program - HAART administers a Ryan White Part B funded program on a statewide basis for the states health department. Staff is responsible for accepting applications, screening for eligibility and assisting more than 3,000 persons and 2,200 persons during 2013 and 2012, respectively, with the cost of health insurance premiums and out-of-pocket costs for co-payments and deductibles for medical services and medications. The Ryan White Grant RW provides primary medical care diagnostic labs X-rays, pharmaceutical assistance, early intervention, medical transportation, and medical non-medical case management services to persons living with HIV/AIDS within the Baton Rouge Metro Targeted Grant Area TGA under Part A. The Minority AIDS Initiative also under Part A and a new grant awarded to HAART in 2012, provides primary medical care and diagnostic services to pregnant minority women living with HIV/AIDS, along with outreach testing early intervention services to at-risk minorities of the general community residing within HAARTs TGA. The Permanent Supportive Housing Initiative PSHI through Title I of the Community Development Block Grant CDBG provides assistance to persons that suffer from mental and/or physical disabilities and who have both a financial and supportive need to obtain and maintain housing to avoid homlessness. The Housing Opportunities for Persons with AIDS Grant HOPWA provides permanent transitional housing assistance to persons living with HIV/AIDS. The Consumer Advocacy Council CAC program is an agency sponsored program providing prevention education through health fairs, group counseling, and other risk management services, and is funded with agency general administrative funds private donations. Assistance with eligible patient 340B dispensing fees and related administrative fees were provided by both HAART and the Caring Clinic as 340B covered entities using savings generated by the operation of the 340B Drug Discount Program. Other Direct Client Assistance to individuals is provided to persons living with HIV/AIDS using agency general administrative funds CDC funds, and private foundation funds. CDC funds along with private foundation HAART general administrative funds provides medical care, prevention and counseling services to at-risk members of the general community who reside within HAARTs TGA. During 2013, 52,948 in CDC, private foundation, HAART general administrative funds were given to individuals through direct client assistance. Form 990, Part III, Line 4d Program Service Expenses 311,885, Grants and allocations 0, Revenue 0 Prevention Serivces - Staff provide HIV counseling, Testing and Referral CTR services, prevention materials availability and evidence-based intervention through the regions only program directly funded by the Centers for Disease Control and Prevention CDC, in partnership with Metro Health Education, another local community-based provider of HIV/AIDS services. The program assisted approx. 340 and 200 persons for 2013 and 2012, respectively. Form 990, Part III, Line 4d Program Service Expenses 2,983,117, Grants and allocations 0, Revenue 0 340B Drug Program - HAART and Caring Clinic are both registered as eligible entities in the 340B Drug Pricing Program, administered by HHS/HRSAs Office of Pharmacy Affairs, and employs three contract pharmacies to manage and dispense medications to eligible patients. The medications are available for these savings. The program filled more than 20,000 and 12,000 prescriptions during 2013 and 2012, respectively, for persons with HIV/AIDS. Form 990, Part III, Line 4d Program Service Expenses 839,489, Grants and allocations 0, Revenue 0 Medical Services - Opened in July 2007, the Caring Clinic of Louisiana, LLC, a wholly owned subsidiary of HAART, provides medical care to more than one-thousand patients with health care insurance or Medicaid/Medicare coverage, including approx. 400 who are living with HIV/AIDS. The Clinics staff includes a Endocrinologist who serves as Medical Director, two family nurse practitioners who treat HIV, two registered nurses and other support staff. The Clinic is located in the same multi-business office building complex as HAART, which contributes to the ability to ensure that clients have access to the additional supportive services it offers. Ryan White Part A funding enables primary care services through Caring Clinic for uninsured HIV positive persongs and Early Intervention Services for those who are newly diagnosed or returning to care. Form 990, Part VI, Section B, Line 11B The Form 990 is reviewed by the Financial Compliance Officer, Administrator, and Executive Director. The reported dollar amounts are traced back to the audited financial statements for accuracy. Form 990, Part VI, Section B, Line 12C Annually, at its first board meeting of the calendar year, the members of the board of directors complete and sign an annual conflict of interest statement disclosing whether or not they are engaged in any activity, either privately or officially, in which a conflict of interest may exist. These certifications are kept on file and compared to previous years certifications for any changes. Agency staff members sign upon employment that they have read, understand and agree to the terms of the policy and have disclosed any conflicts of interest. Upon discovery of any activity which constitutes a conflict of interest, by any agency staff member or member of the board of directors, will result in disciplinary action, not exluding termination of employment or removal from the board of directors, on a case-by-case basis.
