In addition, Access to Independence is committed to community integration and therefore has chosen to concentrate its efforts on transition and advocacy in order to achieve greater life changing outcomes for consumers. In our San Diego and Escondido offices, Access to Independence staff facilitated 92 consumer transitions from institutional settings to community-based living, 41 job placements for consumers with disabilities and assisted 61 seniors with disabilities with financial assistance for housing, Assistive Technology, health-care related and other services which increase access to independent living. In addition, February 2014-June 2014, our staff provided case management and independent living services to 44 consumers referred by a managed healthcare plan. We also hosted 11 Independent Living Skills Trainings, 10 Housing Workshops and 10 Youth Independent Living Skills trainings throughout the year. In Imperial Valley, we assisted 101 consumers and those consumers successfully achieved 63 goals.
