Surveys were sent to representatives from these areas, with responses included in the CHNA Assessment. Following the initial assessment, a smaller group of leaders from these organizations was brought together to work through prioritization and action planning in each service area. Line 4: Riverside Regional Medical Center, Riverside Doctor's Hospital of Williamsburg, Riverside Rehabilitation Institute, Riverside Behavioral Health Center and Hampton Roads Specialty Hospital worked together to complete a CHNA covering the Williamsburg and Peninsula regions. Line 11: Patients eligible under the financial assistance policy were provided 100% discount, and therefore FPG was not used to determine eligibility for discounted care. Part I Line 7, I: Cash and in-kind contributions are the actual dollar value of cash or items given. Part II: HRSH has worked to support the local community. Through the CHNA process, a work group was developed consisting of Riverside employees as well as community leaders. The work group helped identify and prioritize the top health concerns and service gaps, which helped guide Riverside in its Community Benefit funding and support decisions. Additionally, HRSH continued its commitment to educating the next generation of health professionals. This is seen in mentoring and training students, including medical students and residents, nursing students, and therapists. As part of the Riverside Healthcare Association, Hampton Roads Specialty Hospital is consolidated with all facilities on the audited financial statements. The System's financial statements do not contain a footnote specifically for bad debt. Bad debt is referenced in Note 4 community benefit expense. It states "Distinguishing uncollected patient revenue between charity and bad debt requires full consideration of both the financial and non-financial circumstances of the patient, which are not always available to the organization. Therefore, both traditional charity care and bad debt are included as a component of benefits for the indigent." The bad debt expense on the audited system financial statements is bad debt charges written off. This amount is considered a community expense. No expense is listed for line 3. We do not take any patients to bad debt if we know they qualify for one of our financial assistance programs. Even if a charity status is not known but a financial assistance application is pending we do not send the patient's account to bad debt. The policy states "If a patient has completed a hospital's application for financial assistance that account should not be advanced for collection pending determination of eligibility." Line 8: The organization uses CMS cost reporting guidelines and methodology to determine allowable Medicare costs. The shortfall from Medicare should be recognized as a community benefit, since the patients are provided services at a loss for the facility. The Health System is assuring that these essential emergency and acute care services are provided for the population's health. Line 9b: Patients eligible under the written Financial Assistance Policy were provided 100% discount and were therefore excluded from debt collection processes. Line 2: Riverside has embraced the Community Health Needs Assessment process as the way to best assess the health care needs of the communities it serves. To supplement this data, Riverside also utilizes resources such as the Virginia Atlas of Community Health, as well as observing utilization & diagnoses trends across the Riverside service areas. Finally, Riverside works closely with other community leaders to identify and respond to local needs. Line 3: Patients are notified of the FAP upon admission and prior to discharge, notice is included on the patient statement, and patients are educated directly by customer service agents when they inquire about their bill. They are also contacted for assistance in screening for eligibility for federal, state and local government programs. Line 4: Hampton Roads Specialty Hospital primarily serves the residents of the Virginia Peninsula, which is bordered on the east by the Chesapeake Bay and Hampton Roads, on the south by the James River and the north by the York River. Access to health care facilities other than the Peninsula is restricted by tunnels and bridges with three connections to various points across the James and one bridge north across the York. Numerous waterways, inlets and marshland are prominent in the landscape and restrict traffic in certain areas, requiring a system of bridges or circuitous routes for local travel. The hospital was formed to provide specialized care for patients who have medically complex conditions and require longer than expected stays in a more cost-effective setting than an acute care hospital. The primary service area of 480,000 people is generally considered to have a high quality of life in accordance with standard socioeconomic variables, and a comparatively low cost of living. Drugs, crime and poverty negatively impact the more densely populated areas of the community. Demographically and occupationally, the Hampton Roads Specialty Hospital service area is primarily a blue-collar community. The largest employer is Huntington Ingalls. There is a strong military presence in the region in the form of six major bases within a twenty-mile radius. Over 40% of households within the geographic region have incomes below 200% of the Federal Poverty Guideline. Our charity mission is strong and ongoing. Approximately 70% of all patients at Hampton Roads Specialty Hospital are Medicare recipients. Inpatient Medicaid patients account for 5% of patients. The area's population is comprised of 12.9% aged 65 and over. The percent of the population under the poverty guideline is approximately 11.4% while the per capita income is $27,466. The median income is $49,554. Line 5: Hampton Roads Specialty Hospital has served the Virginia Peninsula since 2005. Prior to its opening there was no long-term acute care facility in the geographic area. Patients come from traditional hospitals and medical centers, where they have received short term acute care, more invasive procedures and testing, and have healed beyond the unstable phase of their illness and are moving into their transition to the recuperative stage. The overall leadership and guidance comes from a voluntary Board of Managers, comprised of community leaders. Hampton Roads Specialty Hospital has an open medical staff. To apply, physicians must be board certified or board eligible. Line 6: The hospital administrator, medical director and director of clinical services manages the day-to-day operations of Hampton Roads Specialty Hospital.
