Form 990 Part VI Section B Line 11a The Form 990 is presented to the CEO for review and approval. After CEOs approval, the Form 990 is presented to the Board of Directors for review and approval. The Board approves the Form 990 and the CEO signs the Form 8879-EO, authorizing the preparer firm to e-file the Form 990.
Form 990 Part VI Section B Line 12c Gateway has a Conflict of Interest Policy which it regularly and consistently monitors and enforces. All center providers, staff, and contract staff are required to review their Code of Conduct and sign an acknowledgement of understanding and acceptance.
Form 990 Part VI Section B Line 15a The Governing Board reviews through a performance evaluation and approves the compensation of the CEO. The amount of the compensation of the CEO is determined by comparing the compensation for similar positions in the National Association of Community Health Centers NACHC Compensation Benefits Report. This process was last undertaken in 2005.
