Additional information about The Health Collaborative's assessment process is noted below. In 2013, MHS's implementation strategy, including an execution plan and prioritization of health needs, services and metrics for each hospital were presented to and approved by the MHS Community Benefits Committee, the MHS Community Board, the MHS Board of Governors, and the Methodist Healthcare Ministries Board of Governors. In order to assess the rural areas Methodist Healthcare serves, MHS utilized assessment information from the Regional Healthcare Partnership 6 Plan submitted to the Texas Health and Human Services Commission as part of the Medicaid 1115 Waiver Program. University Healthcare System (RHP6 Anchor) lead this process which included multiple meetings, conference calls and public forums from February - May of 2012. Participants in this process included hospital CEOs, county judges, county commissioners, physicians from the University of Texas Health Science Center at San Antonio, University Health System and Christus Santa Rosa. Input was also obtained from federally qualified health centers, city government officials, indigent care coordinators, advocacy groups and Healthcare Access San Antonio. MHM and MHS are members of the The Health Collaborative which consists the following members: Baptist Health System, Christus Santa Rosa Health System, Bexar County Department of Community Resources, Community First Health Plans, San Antonio Metropolitan Health District (Metro Health), WellMed Medical Management, the YMCA of Greater San Antonio, University Health System, and Community Member, Steve Blanchard, PhD. MHM and MHS are major funders of the Bexar County Community Health Collaborative's needs assessment. As noted above, Methodist Healthcare System is a partner of The Health Collaborative. The Collaborative undertakes a county-wide community assessment study every three years to guide the community health strategic planning process. For the current study, The Health Collaborative engaged Health Resources in Action to collaborate on the process. The goals of the study included: a) To provide a health portrait of the community b) To determine trends and emerging issues, c) To engage the community and key partners to identify health concerns, priorities, strengths and opportunities for future programming and policy. The assessment was comprised of three phases: Phase I - Health Outcomes and Conditions - provided information on Bexar County's social and economic indicators, as well as the leading causes of morbidity and mortality for Bexar residents. Multiple data sources were drawn upon in order to obtain this information and included the U.S. Census Bureau, U.S. Bureau of Labor and Statistics, Texas Department of State Health Services, and the San Antonio Police Department. Phase II - Health Behaviors - this phase examined risk factors and behaviors resulting in the leading causes of morbidity and mortality of Bexar County. Phase III - In-depth Community Health Perceptions - this phase identified those areas requiring further investigation and/or community input. Focus groups, meetings, and interviews were conducted with over 280 individuals. Groups included hospital administrators, WIC Counselors, as well as children and their parents. This phase provided valuable insight into the perceived health needs and assets as well as suggestions for improving the health of the area. Using information from the Collaborative' s Community Health Needs Assessment, as well as other data, Methodist Healthcare System developed individual plans for each campus, which included a 3-Year plan for 2014-2016. The implementation strategy includes the following: communication plan, priority initiative work plan, role and responsibility assignments, and measures/indicators for success along with baseline data. Additional monitoring of the plan will occur through the quarterly community benefits reports to the MHS Community Benefits Committee and the annual charity care report. Part V, Section B, Line 3 As noted above, Phase III of the Assessment identified areas requiring further investigation and/or community input. Focus groups, meetings, and interviews were conducted with over 280 individuals representing different audiences. These groups included faith organizations, social service providers, hospital administrators, local and county government officials, public health leaders, academic researchers, community planning agencies, community focused organizations, individual community members, and business leaders. Part V, Section B, Line 4 The community needs assessment was conducted with the following hospital facilities: Methodist Healthcare System (Methodist Hospital, Methodist Children's Hospital, A Campus of Methodist Hospital, Methodist Specialty and Transplant Hospital, A Campus of Methodist Hospital, Metropolitan Methodist Hospital, A Campus of Methodist Hospital, Northeast Methodist Hospital, A Campus of Methodist Hospital, Methodist Texsan Hospital, A Campus of Methodist Hospital, Methodist Stone Oak Hospital, Methodist Ambulatory Surgery Hospital - Northwest); Baptist Health System (Baptist Medical Center, North Central Baptist Hospital, Mission Trails Baptist Hospital, St. Luke's Baptist Hospital, Northeast Baptist Hospital); Christus Santa Rosa Health System (Christus Santa Rosa Medical Center, Christus Santa Rosa Westover Hills, Children's Hospital of San Antonio) and University Hospital System. Part V, Section B, Line 5c The Community Health Needs Assessment is available on the following websites, and also is available upon request from the hospital: http://sahealth.com, http://www.healthcollaborative.net/ Part V, Section B, Line 12 Methodist Healthcare System uses Federal Poverty Guidelines to determine eligibility. The following is a summary of the Charity Care Policy adopted by MHS: Charity Care Eligibility System - Methodist requires the completion of an application, which allows for the collection of appropriate information. - Verification of family members in the household - Adults: patient, patient's spouse and any dependents. Minors: patient, patient's mother and father, and dependents of both. - Income calculation - Adults: sum of the total yearly gross income of the patient and the patient's spouse. Minors: total yearly gross income of the patient, and the patient's mother and father. - Documentation - various official income reporting documentation is required (e.g. W-2, Wage and Tax Statement, Pay Check Remittance and others). Documentation associated with the participation in a Public Benefit Program can be provided in lieu of income documentation (proof of participation indicates the patient has been deemed financially indigent and therefore is not required to provide income information). There is also a verification process in place for patients that do not have appropriate documentation. - ZIP Code Write-Off Eligibility - Methodist will accept uninsured residential indigent patients as eligible for charity Write-Off upon exhaustion of insurance eligibility determination (i.e. Medicaid) and efforts to obtain a completed Charity application with supporting proof of income. The write-off will apply to all patient types. A residential indigent patient is an uninsured person who is accepted for care with no obligation or with a discounted obligation to pay for the services rendered, and lives in specifically defined ZIP codes--those with high poverty populations. For the year ended December 31, 2013, the charity care write off related to this eligibility process was $86.3 million ($60.7 million in 2012). Charity Eligibility Classifications - Financially Indigent - yearly income is less than or equal to 200% of the Federal Poverty Guidelines. - Medically Indigent - the amount owed by the patient after payment by all third-party payors must exceed ten percent of the patient's yearly income and the patient must be unable to pay the remaining bill. Acceptance by MHS is based on meeting either of two criteria: Yearly income must be greater than 200%, but less than or equal to 500% of the Federal Poverty Guidelines. Alternatively, patients with abnormally large accounts may qualify as catastrophically eligible when their remaining balance exceeds a specific percentage of their income. It is important to note that the guidelines applied for Catastrophic Eligibility range from 201% of the Federal Poverty Guidelines to over 1000% of the Federal Poverty Guidelines. Methodist Healthcare System adopted an Uninsured Discount Policy effective March 1, 2005. The Partnership is structured with two general partners, Columbia/HCA Healthcare Corporation of Central Texas, an indirect wholly owned subsidiary of HCA Holdings, Inc., and Methodist Healthcare Ministries of South Texas, Inc. ("MHM"), a Texas nonprofit corporation, each with a 20% general partnership interest. The two general partners also each hold equal 30% limited partner interests. Part I, Line 7A Explanation - The partnership is a member of the Bexar County Clinical Services (BCCS), as part of an Indigent Care Affiliation Agreement entered into with other healthcare systems in San Antonio. The affiliated hospitals collaborate to ensure the availability of, and to more cost effectively provide, quality healthcare services to low income and needy residents in the Bexar County community. BCCS has contracted with various providers to deliver physician and other healthcare services to the community's low income and needy residents. Each hospital contributes funds to BCCS, which are then used to support the provision of hospital and clinical physician services, physician in-training services, physician assistant/nurse practitioner services, specialty physician services and other healthcare services through BCCS. MHM and MHS jointly analyzed zip codes where, based on financial demographics, it appeared that the patients in those zip codes would likely qualify for charity care. An analysis of past collections in the zip code areas and several federal poverty guideline surveys using the zip codes resulted in MHM's conclusion that all patients in the identified zip codes are patients that, under MHS charity care policies, would likely qualify for charity care. The estimated amount of MHS's bad debt expense attributable to patients eligible under the organization's financial assistance policy was calculated using zip code data. Part III, Line 4 MHS Bad Debt Footnote: Additions to the allowance for doubtful accounts are made by means of the provision for doubtful accounts. Accounts written off as uncollectible are deducted from the allowance and subsequent recoveries are added. The amount of the provision for doubtful accounts is based upon assessment of historical and expected net collections, business and economic conditions, trends in federal and state governmental and private employer health care coverage, and other collection indicators. The provision for doubtful accounts and the allowance for doubtful accounts relate primarily to "uninsured" amounts (including co-payment and deductible amounts from patients who have health care coverage) due directly from patients. Accounts are written off when all reasonable internal and external collection efforts have been performed. The return of an account from the secondary external collection agency is considered to be the culmination of reasonable collection efforts and the timing basis for writing off the account balance. Write-offs are based upon specific identification and the write-off process requires a write-off adjustment entry to the patient accounting system. Management relies on the results of the detailed reviews of historical write-offs and recoveries at facilities that represent a majority of revenues and accounts receivable (the hindsight analysis) as a primary source of information to utilize in estimating the collectability of accounts receivable. The hindsight analysis is performed quarterly, utilizing rolling twelve-month accounts receivable collection and write-off data. The results of the detailed review of historical collections and write-offs experience, adjusted for changes in trends and conditions, are used to evaluate the allowance amount for the current period. At December 31, 2013 and 2012, respectively, the Partnership's combined allowance for doubtful accounts and self-pay discounts represented approximately 96% of the $299 million and 95% of the $257 million total patient due accounts receivable balance, including accounts, net of estimated contractual discounts, related to patients for which eligibility for Medicaid coverage was being evaluated (pending Medicaid accounts). Adverse changes in general economic conditions, business office operations, payor mix, or trends in federal or state governmental health care coverage could affect the Partnership's collection of accounts receivable, cash flows, and results of operations. As of December 31, 2013, the bad debt amount was $126,600,000. This amount is not reflected on MHM's 990 Part IX expenses due to bad debt amount being reported on MHS financials. Schedule H, Part III, Section B, Line 8 The amounts reported on Part III, Lines 5-7 have been determined by aggregating the information from the individual facility cost report(s) for each of the Hospitals operated by MHS. The Hospitals operated by MHS may have cost report year ends other than December 31, 2013. Accordingly, for a facility with a non-calendar cost report year end, the cost report that was filed for the cost report year end that ended during 2013 was utilized. It is important to note that amounts included in lines 5-7 do not include Medicare revenue and related cost for freestanding ambulatory surgery services and for physician services. Schedule H, Part III Section C, Line 9b: MHS has a policy to provide discounts to those individuals who do not have insurance or one not covered by a governmental reimbursement program. If a patient qualifies for Medicaid, then he or she is only responsible for any non-covered charges. If the patient does not qualify for Medicaid, he or she may complete the MHS Financial Assistance Application to have the encounter reviewed for a potential charity discount. If the patient's yearly income is less than or equal to 200% of the poverty guidelines updated annually in the Federal Register by the U.S. Department of Health and Human Services ("Federal Poverty Guidelines"), the patient will be granted classification as Financially Indigent, and the account will be written off to charity. Letters are then sent to the patient notifying that the account has qualified for the charity discount and is considered closed. In addition, a sliding scale discount is applied to accounts for patients whose income is between 200% and 500% of the Federal Poverty Guidelines, and whose remaining account balance, after any third-party payments, exceeds a percentage of their income ("Medically Indigent"). In addition, patients with abnormally large accounts may qualify as catastrophically eligible when their remaining balance exceeds a specific percentage of their income. If a patient does not qualify for a charity discount, an uninsured discount is applied to total charges. If a patient is unable to pay the remaining balance in full, after applying any charity or uninsured discounts, MHS will work with the patient to set up a monthly payment arrangement. Throughout the debt collection process, MHS continues to inform patients about the availability of Charity Care Assistance. Schedule H, Part VI, Question 2 Part V, Section B, Line 1j - discussed The Health Collaborative, its members and the process used to conduct the assessment. Five Key Findings were identified, and those are more fully described below, as well as tactics and/or programs implemented by Methodist Healthcare System to respond to these priorities. Key Finding No. 1: Healthy Eating and Active Living Despite improvements in the physical activity environment, Bexar County still faces challenges in healthy eating and active living. Limited access to healthy foods, an abundance of fast food, and an unhealthy food culture make healthy eating difficult for Bexar County residents. Although the physical activity environment and attitudes are beginning to improve, levels of physical activity have declined in recent years. While population obesity rates have started to decrease, obesity and related chronic diseases are still primary concerns among the Bexar County community. Methodist activities related to this priority included the following in 2013: a) Methodist's community activities include offering a suite of health and wellness programs unlike any other provider in the community - over 2,600 health and wellness events in 2013. b) Over 500 San Antonians participate weekly in Methodist-sponsored line dancing classes throughout the city. c) Two community-wide Heart Check health fairs were held in 2013, one on the west side of San Antonio and the other on the southwest side. Both events attracted over 400 people. d) Methodist offered the following community education classes focusing on prevention--115 classes on Hypertension with 9,331 attendees, 108 classes on Diabetes with 9,642 attendees, and 77 classes on Hyperlipidemia with 8,466 attendees. e) Methodist Healthcare offers a free wellness and prevention magazine called Heart Matters/Stroke Matters that is distributed at all Methodist events. Over 12,000 magazines were distributed in 2013. f) Through Methodist's membership in the Health Collaborative, Methodist plays a large role in the implementation of Project Measure Up, a school-based program that measures the rate of youth obesity and physical fitness of children in grades 3-12. In 2013, Project Measure Up has been implemented in seven of the 16 San Antonio school districts, some of the more disadvantaged areas of San Antonio. Additionally, Fit Family Challenge, a program The Health Collaborative implements for the San Antonio Sports Foundation, served 14,230 individuals in 2013. Key Finding No. 2: Healthy Child and Family Development Teen birth rates are declining across Bexar County. While negative birth outcomes have remained stable overall, African Americans disproportionately experience preterm and low-birth-weight births. Similarly, infant and child mortality have slightly decreased, although African Americans still experience worse outcomes. Methodist hosted more than 900 childbirth/parenting/family classes in 2013. Topics for the classes include Breastfeeding, Beyond Baby Blues and Getting Ready for Childbirth. Methodist offered 278 Buckle Up Baby events/classes in 2013. Parents are taught proper installation of car seats. Methodist distributed 31 car seats to parents who could not afford one. In 2013, Methodist provided 777 CPR Anytime kits to NICU baby families. These kits include a video, booklet and mannequin. In 2013, Methodist held 36 pediatric CPR classes for families. Methodist provided 47,765 lactation consultations (including phone consults) in 2013. Key Finding No. 3: Safe Communities A strong sense of community and a small town feel accompany declining crime rates, although injury and violence have created concern in Bexar County's urban core. In 2013, Methodist performed 745 SAFE (Sexual Assault Forensic Examination) cases. Methodist is involved with Child Abuse Resource Education (CARE) team, an effort involving SAPD, Emergency Medical Services (EMS), local pediatricians and many more to objectively handle and support children and families affected by child abuse. In 2013, Methodist continued its involvement with The Health Collaborative and their program, Young Minds Matter, that addresses bullying and other social and emotional problems among children. Methodist's Sexual Assault Nurse Examiners (SANE) speak to outside agencies about sexual assault. Key Finding No 4: Behavioral and Mental Well Being County leaders and residents view mental health as a critical issue in Bexar County. Economic stress on adults and academic and social pressures on youth have taxed the limited mental health system. Substance abuse was also noted as a concerning public health issue; while tobacco use is declining, alcohol and prescription drug use are viewed as on the rise. Methodist participated in the Bexar County Mental Health Consortium, a blueprint for coordinating and improving mental health services in Bexar County and reducing costs by working with a diverse group of local stakeholders. In 2013, Methodist sponsored the National Alliance on Mental Illness (NAMI) walk. Methodist participates in the Bexar County Consumer and Family Support Conference, a program of the Center for Hope in San Antonio, including in-kind printing donations for the annual consumer conference. (Center for Hope provides mental health, developmental disability and substance abuse services in Bexar County.) Methodist provides an ongoing 24/7/365 Pastoral Care presence with patients, families and health care professionals with chaplain clinicians. In 2013, Methodist provided a memorial service at each hospital, once each quarter, for those person's families who have died during the quarter. Methodist provides bereavement programs such as No One Dies Alone, Service of Leaves and Walk to Remember. Key Finding No 5: Sexual Health Limited sex education in schools and cultural acceptability have led to high teen birth rates, although rates are decreasing among all races/ethnicities in Bexar County. Methodist partners with The Health Collaborative and the HIV Summit. In 2013, 13,918 free pregnancy tests were offered through the five Methodist Family Health Centers. Methodist Healthcare's website, SAHealth.com, offers millions of pages of health resources available to the community, including information on sexual health. In 2013, Methodist offered educational seminars, including topics such as "STDs - How They Affect Us All." Additional Information: In addition to the items noted above, Methodist operates three Health Buses on the east, south and west sides of San Antonio, offering complimentary transportation to various health care facilities. In 2013, 11,599 transports occurred, which is above the mandated benchmark. Another important community service is Call-a-Nurse for Children, a telephone service offering complementary medical advice by trained emergency care pediatric nurses to parents of sick/injured children. The Call-a-Nurse for Children service is open from 5:00 PM to 8:00 AM Monday through Friday and open around the clock on weekends and holidays. Call volumes in 2013 were 84,529. The Methodist Contact Center operates a community phone-in health resource called HealthLine during normal business hours. HealthLine callers schedule attendance to wellness events, parenting classes, and receive physician referrals through the Methodist DoctorSource program. In 2013, Methodist referred physicians to 7,945 callers (up to three physicians may be referred to each caller). In one other community benefits category, Methodist provided direct support to 40 non-profit health and human service agencies serving the community. All community benefit programs are also offered complimentary to affinity membership programs founded and sponsored by Methodist. These membership groups have been in place for over 20 years, and are directed to demographic specific groups including Woman Plus with 364,346 members, 55PLUS with 228,562 members, the Young Heroes Club with 63,981 members (directed at children under the age of 12), and VIVA with 4,171 members (directed to families from Mexico wishing to access care in San Antonio). Total membership increased by 3.4% when compared to 2012. In 2013, Methodist distributed two issues of "Keeping Well", an eight-page news magazine that features news and information about Methodist Healthcare. All content is focused on promoting awards, accreditations, new services, events and expansions at our eight hospitals. "Keeping Well" is distributed by email to 38,000 members of WomanPlus and 55PLUS. Ten thousand copies are printed, of those, 5,000 are mailed to members who have requested a paper copy and the rest are distributed at health fairs, medical office buildings, and hospital lobbies. In late 2011, The Texas Health and Human Services Commission filed an application for a waiver of certain federal Medicaid requirements under section 1115 of the Social Security Act. As a result of the granting of this application the Texas Health Care Transformation and Quality Improvem Schedule H, Part VI, Question 3 MHS has taken steps over the past several years to heighten community awareness regarding MHS's charitable mission. There is a link on MHS website for information on financial assistance. Signs that prominently present information about the charity mission and guidelines are present at all points of admission. MHS also publishes a notice of its charity policy annually in the San Antonio Express-News. A patient brochure, entitled "A Guide to Your Hospital Bill", explains the hospital billing process and informs patients of the charity policy in the event they need financial assistance. Admissions personnel receive training and an annual refresher course on the policy, and understand the crucial role they play in informing patients about the policy. Financial Counselors and Hospital Case Management staff educate patients and are available to assist them with the charity care application process. In addition to providing information during the admitting process, MHS continues to provide information about the availability of charity care assistance during the collection process for those patients who have been billed, but have not paid. MHS adopted an Uninsured Discount Policy effective March 1, 2005. The policy was established to make hospital bills more affordable to the uninsured population. Uninsured patients who qualify for charity will receive either the charity discount or the uninsured discount, whichever is greater. Uninsured discounts granted for the year ended December 31, 2013 were $304.2 million, and are recorded as a deduction of net patient revenue. MHS also partners with Resource Corporation of America (RCA) to screen and to assist self pay patients to determine their eligibility for Federal, State, and County programs. RCA acts as a liaison for the patient to help them through the application process. Patients are referred to RCA in several ways - through review of hospital census reports, review of insurance verification, from physician offices and from hospital case management staff. Schedule H, Part VI Question 4 San Antonio serves not only residents of Bexar and surrounding counties in Texas, but serves as the referral center for all of South Texas--especially for pediatric services, as the outlying areas typically do not offer those services in their communities. Schedule H, Part VI, Question 5 MHS promotes the health of the community through various services it offers to San Antonio and surrounding communities. In 2013, MHS received 27.4% of all inpatient admissions from counties outside of Bexar County, with many of these admissions coming via transfers from rural hospital emergency departments. Since late in 2009 Methodist consolidated all bed placement services, including rural bed transfer assistance, into a system-wide Patient Placement Center. Rural transfers through the Center continue to increase. The acceptance rate in 2013 was 98% compared to 98.7% in 2012. Self-pay patients accounted for 5.2% of all transfers as compared to 5% in 2012. Pediatric and high-risk maternal transfers are also now handled through these consolidated placement services. The acceptance rate for these transfers was 96.9%, with self-pay patients accounting for 2.9% of all pediatric transfers as compared to 3% in 2012. Whether by ground or air, Patient Placement Services is also responsible for assisting in the mode of transportation for transferring patients from rural markets. In mid-2013, Methodist AirCare, in partnership with REACH Air Medical Services, established its second rural helicopter based at Guadalupe Regional Medical Center in Seguin, Texas. Currently, discussions are occurring with two additional rural providers for helicopter base placements. Specialized services available at MHS facilities include the following: Neonatology services, Pediatric Subspecialty services, Bone Marrow Transplant, Kidney Transplants, Heart Transplants, Liver Transplants, Pancreas Transplants, Hyperbaric Oxygen Treatment, Stroke Care, Medical Air Transport, Gamma Knife Radiosurgery, and Bariatric Surgery. See responses for question 2 above regarding expansion of services in 2013. The majority of charity care provided by the hospitals is from patients receiving emergency services. Emergency Department (ED) visits to Methodist facilities totaled 297,047 a decrease of 4% over 2012. Sixteen percent of total ED visits resulted in inpatient admissions with a 3% decrease in ED admissions over 2012. In addition, 23.1% of all visits to Methodist emergency departments in 2013 were made by charity or self-pay patients. EMS volumes were up for Methodist by 1.1% when compared to 2012. Methodist continues to focus on improving access to care for the community by improving emergency department access. When comparing 2013 to 2012, Methodist saw a 1.4% decline in the Patient-Arrival-to-Bed (ATB) metric and an 11.6% improvement in Arrival-to-Greet (ATG) metric. The Average Length of Stay for all patients increased from 187.95 minutes to 199.92 minutes. Improvement in ATG led to a 0.7% drop in the Left-Prior-to-Medical-Screening metric. Texas Transplant Institute (TTI), at Methodist Specialty & Transplant Hospital, a Campus of Methodist Hospital, continues to provide increased access for patients requiring kidney transplants. TTI is the home of the busiest paired exchange kidney transplant program in the nation, and the nation's No. 1 living donor kidney transplant program. MHS also operates three Health Buses on the east, south and west side of San Antonio offering complimentary transportation to various health care facilities. There were 11,599 transports in 2013. Metropolitan Methodist Hospital in conjunction with Methodist Hospital Women's Services, operates five Family Health Centers that provide complimentary pregnancy testing, physician referrals (3,607 in 2013), counseling, health education and screening programs. Visit volumes to these centers decreased slightly when compared to 2012 (17,813 vs. 18,647) as other health care providers have increased their service offerings. MHS also operates Call-a-Nurse for Children, a telephone service offering free medical advice by trained emergency care pediatric nurses to parents of sick/injured children. The service operates from 5:00 p.m. to 8:00 a.m. Monday through Friday and around the clock on weekends and holidays (when physicians offices are closed). Call volumes in 2013 were 84,529.
