Part III, Line 9b - Provisions On Collection Practices For Qualified Patients Prior to discharge, patients who fall under the collection practices receive a Financial application packet to complete with a staff member's business card attached. They are instructed to call if they need assistance in completing the Financial application, which will determine their eligibility for assistance. If eligible, they may qualify for possible charity care allowance. Part VI - Needs Assessment A community needs assessment was started in July 2012 and was completed and approved by the Board in January 2013. The report was issued and dated in March 2013. An implementation plan has been developed.
