Form 990, Part VI, Section B, line 11 The Form 990 was prepared by an independent CPA firm, reviewed by management, and then provided to the board via email prior to filing. Form 990, Part VI, Section B, line 12c Leadership Transformations' (LT) board requires an annual disclosure of conflicts of interest. In addition to this, the finance team monitors all transactions for potential conflicts, and brings any potential conflicts to the President's or board's attention. If a conflict is identified, the independent board votes on whether the transaction is in the best interest of LT, while the person with a conflict recuses his or her self from the decision making process. Form 990, Part VI, Section B, line 15 Compensation and benefits offered to the organization's president and executive officers is determined by the compensation committee of the board of directors. All members of the compensation committee are independent in nature. In determining appropriate compensation packages, the compensation committee references salaries of similar executive positions at other non-profit organizations in the area. This process was last carried out in Nov/Dec of 2013. All compensation decisions are approved by the board and contemporaneously documented in the board minutes.
