Form 990, Part VI, Section B, line 11 Form 990 is prepared by an independent CPA firm and an initial draft of the Form 990 is reviewed by the President, CFO, and Controller for accuracy before the return is filed. The approved draft of the Form 990 is sent to the Audit Committee for review and approval.
