We provide ongoing support services including care management, professional individual and group counseling, mentorship and compassionate caregiving to children, teens, adults and families. Hospice of Santa Barbara provides all its professional services free to the community and receives no government or insurance reimbursements. false false 1374853 Patient Care Services: In the spring of 2014, HSB restructured its programs to provide more streamlined services to its patients and their loved ones by creating a continuum of care in which patients can receive comprehensive care from the time they are diagnosed and move interchangeably among programs depending on what services they need at the moment. Its new Patient Care Services Program opened up to four levels of care: Care Management (for the newly diagnosed patient who may recover from their illness); Palliative Care Management (for either a newly diagnosed patient or a patient in active treatment who is not expected to recover from their illness); Anticipatory Grief (for the loved ones and/or caregivers of a patient living with life threatening illness); and Bereavement Care Management (for the surviving loved one after the death of a patient). Since program restructuring, over 115 new patients have been enrolled accounting for an 18% average month over month growth. Throughout the year, volunteers provided 1,824 hours of service to the community. Patient Care Volunteers offered respite to caregivers and improves quality of life for patients and caregivers with thoughtful companionship, help with household chores and errand, and assisting with transportation to and from medical appointments. Spiritual Care Companions explored the spiritual issues surrounding end of life, provide consultation about meaning, hope, isolation and loss. No One Dies Alone Volunteers provided a reassuring presence to actively dying patients who would otherwise be alone. Hospice of Santa Barbara has been asked to be a constant presence on eight local school campuses providing weekly support groups and is poised to respond immediately to any area school who calls on us when a traumatic death occurs with the campus structure. This program is instrumental in creating the base for successful emotional growth that forms the building blocks needed for life long success.
