A letter of explanation accompanies the questionnaire. The letter illustrates examples of a conflict and reminds the recipient that if any perceived conflict should arise before the next annual questionnaire, the individual is to notify the CEO immediately. The internal audit staff reviews these questionnaires and disclosures each year during the audit process.
