The identity of the persons consulted consisted of members of the Chamber of Commerce, local physicians, community leaders, and business owners. Valley View Hospital Association Part V, Section B, Line 11: The Community Health Needs Assessment identified heart and cerebrovascular disease as an important area to focus on. Valley View is currently expanding the Heart & Vascular Center. Valley View's Cardiac Cath Lab offers diagnostic and interventional therapies to the residents of Garfield, Summit and Eagle counties. A cardiac rehabilitation program, which is medically supervised is a prevention program offered to anyone suffering from heart disease. Different phases of this program are designed to educate on healthy dietary habits, stress management and coping skills. In 2015 the new Willits and Battlement Mesa Heart & Vascular Center offices are expected to open to reach more members of the community. Valley View is motivated to improve the community's awareness of the importance of maintaining healthy habits. Implementation of this goal includes Health Fairs which offer several free screening procedures as well as some screening and tests that are available for a fee. The free screening services include: skin, foot, dental, posture & flexibility, vision/glaucoma, breast, body composition, hearing, immunizations, BMI, and blood pressure. By offering these free services, Valley View hopes to promote general health and wellbeing. Along with these services, free events, such as lectures by doctors on healthy habits are occasionally offered. Part II, Community Building Activities: Part II, Line 2: The Hospital creates new employment opportunities in areas with high rates of joblessness. Part II, Line 3: The Hospital provides mentoring programs for troubled teens in the community. Part III, Line 3: The Hospital used a cost to charges methodology in deriving bad debt expense (at cost) attributable to patients eligible under the Hospital's charity care policy. Analysis was done on randomly selected accounts written off to bad debt. In approximately 88% of accounts tested, patients would have qualified for charity care but refused to provide information and documentaion for charity assessment, and were therefore included in bad debt. Part III, Line 4: Accounts receivable are reduced by an allowance for doubtful accounts. In evaluating the collectability of accounts receivable, the Hospital analyzes its past history and identifies trends for each of its major payor sources of revenue to estimate the appropriate allowance for doubtful accounts and provision for bad debts. Management regularly reviews data about these major payor sources of revenue in evaluating the sufficiency of the allowance for doubtful accounts. For receivables associated with services provided to patients who have third-party coverage, the Hospital analyzes contractually due amounts and provides contractual allowances based on these amounts. Additionally, an allowance for doubtful accounts and a provision for uncollectible accounts is provided for expected uncollectible deductibles and copayments on accounts for which the patient is responsible. The difference between the standard rates (or the discounted rates if negotiated) and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts.The Hospital's allowance for doubtful accounts was 17.5% and 17.2% of accounts receivable at December 31, 2014 and 2013, respectively. In addition, the Hospital's write-offs were $5,179,000 and $6,614,000 for the years ended December 31, 2014 and 2013, respectively. Part III, Line 8: Valley View Hospital is the hospital of choice for the elderly population because medical staff in the surrounding area resists taking care of this population. The Hospital used a cost to charge methodology in deriving Medicare allowable costs. Part III, Line 9b: The Hospital provides care without charge or at amounts less than its established rates to patients meeting certain criteria under its charity care policy. Part VI, Line 2: The Hospital assesses the health care needs of the communities it serves through surveys taken by community health departments and input from community focus groups.
