<Desc>La Plata Youth Services (LPYS) was founded in 1994 to provide a continuum of services to at-risk youth and their families including community-based prevention, early intervention, and crisis intervention services. LPYS promotes positive youth development and encourages community engagement among youth at risk of becoming involved in the criminal justice system. Additionally, we work to reduce recidivism, substance abuse, out-of-home placement, truancy, and academic failure. At LPYS, youth are supported, coached, and monitored in a variety of ways with the goal of providing community accountability while building skills to support healthy behaviors and school success. Since 2001, LPYS has been designated as the single point of entry for all youth criminal diversion programs in La Plata County.LPYS programs focus on the interests, strengths, risk factors, and individual needs of each adolescent to instill a sense of accountability and self-worth. We believe in restoring a youths community connections through meaningful community service, structured family support, and collaboration with volunteers and like-minded organizations in La Plata County. Professional staff provides services to youth while working closely with their families, school employees, law enforcement, and victims when appropriate.LPYS programs and services are driven by and monitored through our assessment process. Every LPYS client participates in the assessment. LPYS uses the Insight to Impact 360 assessment tool, created by Youth Zone in Colorado Springs, to assess risk and then guide positive growth in five areas: Alcohol, Tobacco and Other Drugs; Optimism and Problem Solving; School and Community Involvement; Delinquency and Aggression; and Self-Deprecation. The Insight to Impact assessment is a three part process: Intake, Progress, and Completion. Intake is the initial assessment of risk areas, relationships, personality, hobbies, etc. The Progress Assessment is continuous on how they progress on the programs and obligations as laid out in their contract. The Completion Assessment indicates the overall achievement of completing their contract requirements, and shows any improvements in the defined risk areas. The contract developed between the youth and Case Manager includes a client-specific plan to guide positive growth in each risk area.Truancy Prevention and Early Intervention Program: La Plata Youth Services provides services to area youth as a strengths based alternative to truancy court. In addition to providing truant youth with assessment and case management services, LPYS is working to develop a model of care to address the specific needs of truant children and youth. This model includes the development of SMART the School Multidisciplinary Assessment Review Team. Starting in the fall of 2013, in collaboration with the 6th Judicial District Judge and 9-R School District, La Plata Youth Services formed SMART to provide wrap-around care to complex need youth and their families and address the underlying factors contributing to school attendance issues. SMART involves membership of 10 core members including: La Plata Youth Services, Durango School District 9R, Bayfield School District 10 JT-R, Ignacio School District 11 JT, La Plata County Department of Human Services, Axis Health System, Durango Police Department, Bayfield Marshals Office, 6th Judicial Court Senate Bill 94 Pre-Trial Services, and Animas High School. The SMART Coordinator is an employee of La Plata Youth Services. In February of 2014, LPYS received a grant from the Division of Criminal Justice to support prevention and early intervention services for youth with attendance issues and expand services to elementary aged children. The result has been the creation of a youth informed model in one middle and one elementary school that is providing mentorship, case management, mental health services, academic support, and transportation critical services to high-need children, youth and families. Total Youth Served: In our most recent fiscal year ending June 30, 2015 (fy15), La Plata Youth Services received 149 referrals to our diversion and truancy programs. Of those referrals, we provided intake and screening services to 109 duplicated, and 93 unduplicated youth. Of the youth who received screening services, 80% were diversion program referrals 41% were referred by the courts in La Plata County (municipal, county and district) as an alternative to formal court processes, and 39% were referred by law enforcement agencies on a pre-file basis. 12% of the youth referred in fy15 who received screening services were referred by schools into our truancy program. The majority of youth we served with intake and screening services in fy15 were referred for marijuana (28%), alcohol (10%), theft (including shoplifting) (25%), and truancy (12%) related offenses.149 total referrals: 51 court referrals, 45 law enforcement referrals, 38 school-based, truancy referrals, 15 parent/self and other referrals.108 duplicated and 93 unduplicated youth completed intakes, including receiving assessment services, as compared to 105 (unduplicated youth) in fy14, and 86 in fy13; 86 youth referred by law enforcement or court, or 90% of law enforcement referrals; 13 youth supported for truancy intervention with intake and case management services, or 34% of school-based truancy referrals.79 youth served in summer program (duplicated). Youth in summer program contributed over 150 hours of community service through service learning projects. Programs Offered:Individualized case management and youth advocacy: At the heart of our programming, Youth Advocates conduct assessments and clinical interviews and create, monitor, and support case plans. We serve as advocates for youth in the larger community, "connecting the dots" between schools, families and local organizations.Useful public service/service learning: LPYS partners with other organizations locally to give youth choice in how they give back. Service allows youth to see their intrinsic value as members of our community.Substance abuse and other teen education groups: Groups are offered depending on need, and our most utilized group intervention is a five-week cognitive behavioral therapy group that provides adolescent marijuana users with tools to reduce or eliminate their use. The group helps teens take stock of their use and how it affects their lives. Participants also receive concurrent, one-on-one motivational interviewing sessions in order to support their change goals.Restorative justice: LPYS maintains a number of volunteer community conferencing specialists in the community who provide mediation to victims of juvenile crime in our community.Summer programming: LPYS has developed a robust summer program designed to support at-risk youth in continual learning, positive community engagement, and leadership skills development.Parent coaching: Parents may be referred for one-to-three individual coaching session, concurrent with their childs engagement in services.Collaborative programming: LPYS serves as the lead agency for the School Multidisciplinary Assessment Review Team (SMART), a community collaborative providing a wrap-around intervention response to the issues facing chronically absent and at-risk youth and their families. Services include:Community-in-school partnerships Two elementary and two middle schools participate in this pilot program, focused on delivering prevention/early intervention services to students facing significant, adverse barriers to school attendance. Programming focuses on improving school climate and coordinated delivery of community resources in the school setting.Therapeutic mentorship In partnership with the University of Denver, SMART will launch a robust mentorship program in the 2015-16 school year. Masters of Social Work interns each support 2-4 high-risk youth throughout the school year through mentorship, therapeutic intervention, skills development, academic support, and advocacy.Success RatesBuilding positive behaviors: The majority of youth served in fy15 participated in the program for a period of 4.5 months. Aggregately, the youth we served in fy15 saw an overall increase in positive behaviors. This included a decrease in reported alcohol and other drug use, an increase in optimism and problem solving skills, an increase in school and community involvement, and a decrease in delinquent and aggressive behaviors. Program completion In fy15 80% of the youth who completed the program were successful. In our diversion program, this means a youth completed the terms of their contract, and the charges initially filed against them were dropped (if referred on a pre-file basis by law enforcement), or were dismissed (if referred from court). In instances when a youth is referred to our program by a school for truancy, a status offense, a youth is considered successful when they re-engage in school and attendance improves. Redu</Desc>
