The organization experienced a change in board and management leadership and needed to reorganize itself into a functioning organization. After the new Executive Director took charge of the organization, she quickly realized that this form had not been filed and engaged a CPA to complete the form as quickly as possible. There was no wrongdoing or fraudulent activity and the organization staff continued performing its exempt function and exercising internal controls over assets until the leadership situation was resolved in the late winter of 2016.
