<ExplanationTxt>PART V, SECTION B, LINE 11: THE IMPLEMENTATION STRATEGY IS AVAILABLE TO THE GENERAL PUBLIC UPON REQUEST AND ON OUR WEBSITE.BASED ON THE RESULTS OF THE COMMUNITY HEALTH NEEDS ASSESSMENT, A PRIORITIZATION WORKSHEET WAS COMPLETED BY THE CHNA STEERING GROUP. THE HOSPITAL IS WORKING TOWARD MEETING IDENTIFIED NEEDS. PRIORITY 1 IS PHYSICIAN RECRUITMENT. THE FACILITY IN 2014 SUCCESSFULLY RECRUITED 2 PHYSICIANS TO THE AREA. THE PHYSICIANS CONTINUE TO BUILD THEIR PRACTICES IN 2015. WE HAVE ENGAGED RECRUITING FIRMS TO RECRUIT ONE MORE FAMILY PRACTICE PHYSICIAN AS OUR LONG TIME PHYSICIAN IS LOOKING TO RETIRE IN 2017. WE ARE ALSO RECRUITING AN ADDITIONAL PHYSICIAN ASSISTANT/ CERTIFIED NURSE PRACTITIONER. PATIENT VISITS PER CLINIC PROVIDER HAVE INCREASED FROM 1,228 IN 2013 TO 1,324 IN 2014 AND 1,682 IN 2015.AS FAR AS VISITING/SPECIALTY PHYSICIANS WE ADDED AN ORTHOPEDIC PHYSICIAN LATE IN 2015. THIS PHYSICIAN WILL IN THE FUTURE PERFORM COMPLETE KNEE AND HIP REPLACEMENTS. INITIAL MEETINGS WITH THE COMMUNITY HAVE BEEN VERY POSITIVE IN REGARDS TO THIS SERVICE. ORTHOPEDIC SPECIALTY VISITS HAVE INCREASED 13.5%. WE ADDED A SEDATION DENTIST IN 2014 AND HE CONTINUES TO GROW HIS BUSINESS. DENTIST VISITS INCREASED 6% IN 2015. OUR VASCULAR SURGEON VISITS HAVE IMPROVED FROM 12 IN 2014 TO 125 IN 2015 AS HE STARTED HIS BUSINESS LATE IN 2014. WE ADDED A HIGH RISK OB/GYN PHYSICIAN IN 2015. THIS PHYSICIAN HAD 54 VISITS DURING THE YEAR. OVER ALL WE HAVE INCREASED SPECIALTY SERVICE VISITS 11.8% OR 200 VISITS FROM 2014 TO 2015. THESE SERVICES ALLOW OUR COMMUNITY MEMBERS ACCESS TO SERVICES CLOSE TO HOME. THE FACILITY IS ACTIVELY RECRUITING A PAIN DOCTOR, NEUROLOGY, UROLOGIST, AND ONCOLOGIST TO ADD TO OUR SPECIALTY CLINIC SERVICES. IN ADDITIONAL WE HAVE UPDATED OUR SPEECH CONTRACT WITH OUR LOCAL THERAPIST. THIS WILL ENHANCE OUR SERVICES TO OUR INPATIENTS AND NURSING HOME PATIENTS. ALSO THIS WILL INCREASE THE COMMUNITY'S OPPORTUNITY FOR SPEECH THERAPY SERVICES. WE HAVE HIRED A PHYSICIAN THERAPIST TO START OUR THERAPY SERVICES IN AN EFFORT TO PROVIDE A SMOOTH TRANSITION BACK TO HEALTH FOR OUR ORTHOPEDIC PHYSICIAN'S PATIENTS. WE PLAN TO HIRE ADDITIONAL THERAPIST AS THE BUSINESS GROWS. UNDER THE PRIORITY NUMBER 2 THE FACILITY DEVELOPED A WELLNESS COMMITTEE OF SEVEN PEOPLE. THE COMMITTEE HAS MET TO PROMOTE EMPLOYEE-WELLNESS PROGRAMS TO OUR WINNER REGIONAL EMPLOYEES. SPECIFICALLY THE COMMITTEE DECIDED ON THREE AREAS TO FOCUS ON FOR OUR EMPLOYEE WELLNESS: SMOKING, FLU PREVENTION AND A DRINKING AND EATING HEALTHY PROGRAM. THE PROGRAMS WERE DISSEMINATED AT OUR QUARTERLY ALL STAFF MEETING. STAFF WAS INFORMED OF SUCCESSFUL SMOKING SECESSION PROGRAMS PROVIDED BY THE STATE OF SOUTH DAKOTA. HANDOUTS AND SUPPLIES WERE DISSEMINATED TO STAFF. PARTICIPATION WAS ENCOURAGED BY THE WELLNESS COMMITTEE. THE FACILITY IMPLEMENTED A FLU SHOT POLICY. ALL PROVIDERS AND EMPLOYEES ARE REQUIRED TO RECEIVE A FLU SHOT REGARDLESS OF THE AMOUNT OF PATIENT INTERACTION. THIS WAS IMPLEMENTED FOR OUR PATIENT'S SAFETY AND TO SET AN EXAMPLE TO THE COMMUNITY THAT WE CARE ABOUT OUR PATIENTS AND OUR COMMUNITY'S HEALTH AND WELLNESS. OUR DIETICIAN EXPLAINED THE EATING HEALTHY PROGRAM AND PUT TOGETHER A PICTORIAL OF THE EFFECTS OF EATING HEALTHY. THE PICTORIAL WAS POSTED IN THE CAFETERIA TO BE VISIBLE FOR THE FACILITY'S STAFF AND THE COMMUNITY TO SEE AS THEY DECIDE ON THEIR FOOD CHOICES. UNDER PRIORITY 3 CONSTANT TRANSITIONS IN KEY POSITIONS HAS SLOWED SIGNIFICANT PROGRESS ADDRESSING CHRONIC HEALTH ISSUES. OUR YEARLY HEALTH FAIR IN JUNE PROVIDES WELLNESS CHECKS, HEART SCREENS AND STRESS TESTS AT REDUCED COSTS FOR OUR COMMUNITY MEMBERS. WE COMPLETED 151 WELLNESS CHECKS THE CLINIC OFFERS REDUCED PSA TESTING YEARLY IN SEPTEMBER. WE COMPLETED 54 TESTS THIS YEAR. MOVING FORWARD WITH LONG TERM STAFF IN PLACE AND NEW DATA WE CAN IMPROVE OUR FOCUS ON THE CHRONIC HEALTH ISSUES.</ExplanationTxt>
