Participating community members were asked one question What do we need in our county to make it a healthy place to live, work and play. With 1,756 responses approximately 3-4 responses per person, the feedback obtained through the visioning process was varied. The responses were first condensed into broad strategy categories. Group Jefferson Memorial Hospital Line Part V, Section B, Line 3 These categories were initially established by the number of times specific types of strategies were suggested. Community Health Needs Assessment CHNA stakeholder and representatives from Health and Human Services Collaborative HHSC were asked to further select the top priority areas for intervention. Both groups identified the same top priorities for the region, although the rank order was different between the two groups. The CHNA top priority areas were 1. Chronic Disease 2. Cancer lung and breast and 3. Behavioral Health/Substance abuse. Group Jefferson Memorial Hospital Line Part V, Section B, Line 4 The CHNA was undertaken jointly with Berkeley Medical Center. Group Jefferson Memorial Hospital Line Part V, Section B, Line 7d Together Jefferson Medical Center and Berkeley Medical Center prioritized the community health needs identified in the Assessment and decided to focus on the top 3 issues identified in the CHNA 1. Chronic Diseases/Illness, 2. Behavioral Health/Substance Abuse, and 3. Cancer in particular lung and breast. Our implementation plan was based on addressing these 3 issues. Maternal/Child Health and access to better, healthy food choices community gardens, farmers markets were both identified as well. Although those issues were not included in the implementation plan, Berkeley Medical Center did recently open a NICU and the WVU Health Sciences Center Eastern Division and University Healthcare collaborated with local farmers markets to obtain a federal grant to improve access to these products for residents in the Eastern Panhandle. Group Jefferson Memorial Hospital Line Part V, Section B, Line 13b Jefferson Medical Center offers discounted care only to individuals who qualify for free care. JMC offers a Prompt - Pay Discount Program - patients who have no third party coverage uninsured or uninsurable may be eligible for a 50 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To calculate net expense, bad debt of 3,412,249 was deducted from total expenses of 50,359,009 as shown in Part IX line 25 of the core Form 990. Part I Line 7 Worksheet 2 from the IRS Schedule H instructions was used to derive the Cost-to-charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost. Part II Physical improvements and housing - Jefferson Memorial Hospital employees spent a weekday volunteering at three local social service agencies on behalf of the United Way of the Eastern Panhandle Boys Girls Club, Eastern Panhandle Care Clinic, and Hospice of the Panhandle. Part II Economic Development- A representative from JMH serves on the Jefferson County Chamber of Commerce Board of Directors. Part II Community Support- We sponsor and have a team participate in the annual March of Dimes Walk Jefferson County, American Cancer Society Relay for Life in Jefferson County and various other fundraising events for local, health related, non-profit organizations. Many WVUH-East employees serve as volunteer board directors on community non-profit organizations such as Hospice of the Panhandle, Boys Girls Club of the Eastern Panhandle, Eastern Panhandle Free Care Clinic and the United Way of the Eastern Panhandle as well as participate in other non-profit organizations such as the Jefferson County Council of Aging Senior Center, FLOC, provide children the opportunity for hands on learning through programs and youth development, EPIC, an autism awareness and advocacy group and Young Lives, a group that provides mentoring, referrals to community resources, and life skills training to teenage moms. Part II Leadership Development and Training - One manager participated in the Chamber of Commerces Leadership Jefferson program for 2015. Several managers participated in leadership development trainings and have sent several staff to services to enrich their skills. Part II Coalition Building - JMH staff participates in the Health and Human Services Collaborative and workgroups such as the Health Work Group and Behavioral Health Work Group as well as Community Health Initiative- Healthier Jefferson County. These activities focus on improving community access to health care, human services in general, and determine ways to encourage healthy lifestyles. JMH also sponsored health wellness events and participated in wellness events with the local schools, Jefferson County Parks and Recreation, the city of Ranson, the City of Charles Town through its Discover Downtown program and employer wellness fairs such as American Public University, Hollywood Casinos and the IRS. Part II Community health improvement advocacy - WVUH-East is also a partner in the MAPP project which is a collaborative initiative to assess community social services needs and develop a plan to tackle these issues. In 2015, Jefferson Memorial Hospital produced the final community health needs assessment report and is in the process of developing implementation plans to address four health needs in the community- chronic disease, cancer lung and breast, and behavioral health/substance abuse. JMH staff participate on wellness committes for the Jefferson County school board as well as for the individual schools. JMH staff also participate in ATAC, Tobacco Awareness Coalition, and sponsored a food program at the Jefferson County Community Ministries, another local nonprofit agency, promoting healthy eating and teaching healthier cooking. Part II Workforce Development- WVUH- East is also a major sponsor of the Womens Network, which is a new organization, established by the Berkeley and Jefferson County Chambers. This group sponsors womens professional activities, social networking and professional development seminars. Jefferson Memorial Hospital also has agreements with area community colleges and universities to allow students to complete clinical rotations at the hospital. JMH also provides a job shadow program and a summer junior volunteer program to teens to allow them opportunities to shadow professionals to learn about possible careers in the workforce. JMH also participates in school careers fairs in the elementary through high schools in the county. Part III Line 2 Bad Debt Expense at Cost was calcuated by multiplying bad debt expense of 3,412,249 by our cost-to-charge ratio of 52.57 for a total of 1,793,819 less DSH allocations to self-pay patients of 1,139,217 for a total of 654,602. Part III Line 2 The allowance for doubtful accounts is estimated based upon a periodic review of the accounts receivable aging, payor classifications, and application of historical write-off percentages. Jefferson Memorial Hospital grants credit without collateral to its patients, most of whom are local residents and are insured under third-party agreements, primarily with Medicare, Medicaid, and various commerical insurance companies. Jefferson Memorial Hospital maintains allowances for potential credit losses and such losses have historically been within managements expectations. Part III Line 3 Estimated bad debt attributable to patients eligible for charity care was calculated by running a report within our patient revenue software of all bad debt account balances greater than 15,000. The total of that report was 31,416 which we then multiplied by our cost to charge ratio of 52.57 for a total of 16,522. Part III Line 3 We feel that at a minimum the estimated bad debt attributable to patients eligible for charity care of 16,522 should be considered community benefit due to the fact that anyone with outstanding balances of 15,000 or greater usually qualifies as catastrophic if the patient completes the application process. We provide care to individuals in need without regard to ability to pay. Part III Line 3 In our charity care policy, we define catastrophic care as any illness or injury that will likely require continuous or frequent treatment for more than one year. Part III Line 4 Jefferson Memorial Hospitals financial statements are prepared on a consolidated basis as a member of WV United Health System. The footnote for Accounts Receivable - Patients are reported at net realizable value. Accounts are written off when they are determined to be uncollectable based upon managements assessment of individual accounts. In evaluating the collectability of patient accounts receivable, the System analyzes its past history and identifies trends for each of its major payor sources of revenue to estimate the appropriate allowance for doubtful accounts and provision for bad debts. For receivables associated with services provided to patients who have third-party coverage the System analyzes contractual amounts due and provides an allowance for doubtful accounts and a provision for bad debts, if necessary. Part III Line 4 For receivables associated with self-pay patients, which includes both patients without insurance and insured patients with deductible and copayment balances, the System records a significant provision for bad debts in the period of service on the basis of its past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the billed rates and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts. Part III Line 8 The amount reported in Part III Line 6 17,056,799 was calculated using the total Medicare allowable cost from the Medicare cost report. Part III Line 8 Jefferson Memorial Hospitals 3,067,890 shortfall of Medicare program reimbursement should be considered a community benefit because we are relieving a government burden by providing care in excess of our costs to these patients. Part III Line 9b JMH does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity, the patient will not be sent to a bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy. Part VI Line 2 Jefferson Memorial Hospital is a critical care hospital within the health care system of West Virginia University Hospitals - East .The Marketing Department considers several components in assessing how the organization determines the need of the communities it serves. Through outside consultants to collaborating with various departments, a needs assessment takes into account several factors including physician to population ratios, physician availability in the entire service area, and general health risks for our community. The Primary Service Area PSA for WVUH-East includes Berkeley County, WV and Jefferson County, WV with Morgan County, WV being a secondary service area. This data is then analyzed to provide general estimates to where we could better care for residents of West Virginias Eastern Panhandle. Part VI Line 2 Sources utilized to help gather this information come from a variety of areas. Physician distribution and population data come from the American Medical Association and the U.S. Census Bureau and the general health risk for our community come from clinical diagnosis information gathered by the West Virginia Health Care Authority and Thomson Reuters Market Planner Plus. We also conduct market research to ask consumers how they perceive the health system and where they go for health care services. Once all the data is gathered to determine where residents of West Virginia are going for care and what types of services they are receiving, a needs assessment can then begin based on physician distribution and population estimates. Note that WVUH-East is working with other local organizations in a project known as MAPP Mobilizing for Action through Planning and Partnerships to create and implement a community health improvement plan. Part VI Line 3 JMH employs 2 financial analysts to meet with patients to discuss financial eligibility for our charity care program. JMH has applications for charity and the qualification requirements for receiving charity available at all registration areas for both facilities. Patients who do not qualify for state or federal funded programs can apply for the charity care program at WVUH-East. Part VI Line 4 Jefferson Memorial Hospital is a non-profit critical access hospital located in Jefferson County, WV population 56,482 in the city of Ranson with a total population of 4,830 residents. Jefferson Memorial Hospital along with City Hospital, Inc. located in Martinsburg makes up WVUH-East which is a part of West Virginia United Health Systems. Part VI Line 4 WVUH-East provides the majority of inpatient services for the population in our Primary Service Area PSA. The PSA for WVUH-East includes Berkeley County, WV, and Jefferson County, WV. The 2014 market share for Jefferson Memorials Primary Service Area is 8.7 with 28.7 of those patients covered by Medicaid and 3.0 of the patients uninsured. City Hospital, Inc., a non-profit acute care hospital under common management, is within the Primary Service Area and War Memorial Hospital, a non-profit critical access hospital, operated by Valley Health System, is in the extended service area. Part VI Line 4 Jefferson Countys population is 56,482, with an average household income of 66,205 and an unemployment rate of 4.1. Currently, 10.2 of Jefferson County residents live below the poverty line according to the US Census Bureau 2010-2014. Berkeley County has a population of 111,901, an average household income of 55,100 and an unemployment rate of 5.0. Currently 13.2 of Berkeley County residents live below the poverty line. In University Healthcares extended service area, Morgan County, WV has a population of 17,524, with an average household income of 37,406 and unemployment rate of 5.5. Currently, 13.0 of Morgan County residents live below the poverty line. Part VI Line 5 The JMH board of directors is a community board, with fourteen of the fifteen members living in or around Jefferson County. The remaining board member lives in Morgantown, WV and has connections with JMH through the WV United Health System. Having members living outside our PSA allows us to be more aware of the healthcare needs in other areas of West Virginia. None of the board members are employees or contractors of the filing organization, or family members thereof. Part VI Line 5 JMH extends privileges to all qualified medical staff in Jefferson County and surrounding communities to meet its healthcare needs. JMH allocates available funding to capital purchases and expanded services to improve patient care, support medical education in Jefferson County and surrounding counties in West Virginia and bordering states. Part VI Line 5 In 2009, WVUH-East started offering discounted screening in response to a struggling economy. Recognizing that individuals were putting off scheduling routine, preventive testing such as mammograms and blood work, WVUH-East offered Discounted Mammogram Clinics in May and October 2012 cash, no insurance. The clinics were successful in serving over 50 patients at Jefferson Memorial Hospital. Part VI Line 5 Our Mini-Medical School Program, which is held 8 times a year in Berkeley County, is a community health education program featuring local physicians and topics that address health needs in our communities. The program is jointly sponsored by WVUH-East and the WVU Robert C. Byrd Health Sciences Center Eastern Division. It is offered free to the community with an average attendance of 75 - 100. Part VI Line 6 JMH is a part of the WV United Health System. JMH plays a significant role in improving the general health care of the community. The strategic plan of the System states intent to build a regional health care delivery system in its services area, defined above, while offering a variety of options for providers who want to participate. The System maintains a demonstrated commitment to assist rural communities in preserving and improving the health care available to the patients it serves. System management is focused on recruitment of staff and employees to meet the growing needs of the aging population in the Systems services areas. Other hospitals in the System include United Hospital Center, which is more centrally located in the state City Hospital, Inc. which is located in the Eastern Panhandle of West Virginia West Virginia University Hospital located in Morgantown, WV Camden-Clark Medical Center in Parkersburg, WV Potomac Valley Hospital in Keyser, WV and St. Josephs Hospital of Buckhannon, Inc. in Buckhannon, WV.. St. Josephs joined the System in October 2015. In addition to these practices, the System also includes WVUH-East, Inc. and Camden-Clark Health Services which operate as management companies for their respective hospitals.
