In addition to educating and facilitating safe bike riding for both the children and their families, the grant allows the hospital to incorporate a more structured educational component by providing bike riding helmets to children in need, as well as educational coloring and activity books, informational brochures and pencils and stickers. Parents are provided with educational brochures/hand-outs on bike and helmet safety for their children and to use as guidelines for themselves. A grant was made to the hospital to support the graduate medical education of pediatricians and pediatric specialists. The hospital is an established teaching hospital and comprises the largest free standing pediatric training hospital in the Southeastern United States. The Primary Care Pediatric Residency Training Program is structured in accordance with the requirements of the American Board of Pediatrics. All facets of pediatric care are encompassed in the training program from the complex intensive care to the office practice of general pediatrics. Each year, medical students and scholars from all parts of the U.S and around the world apply for acceptance to the Hospital's Primary Care Pediatric Residency Program, which has been in operation for over forty-five (45) years. A grant was made to the hospital to support their Volunteer Services Clown Care Program. This program hires clowns that visit many areas of the hospital to provide activities and entertainment to the hospital patients. The draft tax Form 990 will be mailed to Finance Committee of MCHSF Board of Directors for review and comments prior to the regularly scheduled meeting. 2. The draft tax Form 990 will be reviewed with the Finance Committee. 3. Questions will be answered and if necessary, tax forms will be adjusted. 4. The final Form 990 will be posted on the MCHSF Board of Director's web portal. Form 990, Part VI, Line 12c Conflict of interest policy All Miami Children's Health System Foundation employees are informed upon hire, and on an annual basis thereafter, that they have an affirmative duty to disclose any actual, potential or perceived conflicts of interest. They are required to complete a conflict of interest disclosure form upon hire, annually and whenever a change in circumstances warrants. Conflict of interest disclosure forms are forwarded to the Compliance Department by the Talent Management and Effectiveness Department. These forms are initially reviewed by Compliance Department Staff. When deemed necessary, the Compliance Department collaborates with the Legal Department. Conflicts of interest that necessitate Board awareness and action are presented to the Audit and Compliance Committee of the Board for resolution and imposition of sanctions. If warranted, in addition, on an annual basis, Board Members, Principal Officers and Board-Delegated Committee Members sign a statement which affirms that each individual: (A) Has received a copy of the Conflict of Interest Policy; (B) Has read and understands the Policy; (C) Has agreed to comply with the Policy; and (D) Understands that the Foundation is a charitable organization and that in order to maintain its federal exemption it must engage primarily in activities which accomplish one or more of its tax-exempt purposes.
