We provided free, nutritious meals and holistic programs that impact seniors' physical, emotional and spiritual well-being at 11 senior centers in low-income communities; care for older adults living with Alzheimer's disease or other forms of dementia at our Adult Day Center; evidence-based health promotion classes that improve seniors' health and wellness; and assistance with home managements and personal care through our Home Care program.In 2015, we provided 2,131 seniors with 180,461 hot, nutritious meals; 170 individuals with dementia and 357 caregivers were served at the Adult Day Center; 690 seniors participated in Health Promotion courses; 104 seniors received 15,941 hours of home care and volunteers checked on 118 seniors each day; and 325 older adults received case management and 119 family caregivers were served. A copy is provided to the Finance and Administration Committee for review and approval. Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts As stated in the Agency's Board of Directors' Policies, "Each Board Member must disclose any relationship from which a conflict may arise. Board Members, Officers and certain employees are required to complete an Annual Disclosure Statement to report all actual, perceived and potential conflicts of interest that have arisen as a result of their service or employment with the Agency.
