Davis hospital in Sacramento, to assist patients in establishing or reconnecting to a medical home instead of visiting emergency departments for primary or episodic health care. Since July 2013, patient navigators have facilitated more than 13,000 clinic visits for patients previously seen in emergency departments for their primary health care. The success of the program, thus far, demonstrates its great potential to further reduce readmission rates among the uninsured and under-insured patients seen at Dignity Health and U.C.
