Pt I Line 2 First, SFFCM acts as a fiscal sponsor for, and receives contributions on behalf of, small music ensembles which are too small to have established their own separate IRC Section 501(c)(3) status ("affiliates"). When contributions are received on behalf of a particular organization, SFFCM earmarks the funds for that organization. The organization may request disbursement of the funds, in writing, for specific purposes in furtherance of its artistic projects and mission. Pt I Line 2 Second, SFFCM administers the Musical Grant Program. SFFCM raises funds from local foundations for the purpose of re-granting such funds to ensembles or individuals whose projects meet the guidelines of the program. Award recipients are selected by an independent panel whose members base their selections on written proposals and work samples submitted by the applicant. Panelists are musicians, educators, or other individuals with expertise in musical matters. No panelist is affiliated with SFFCM. Upon receipt of a signed contract, SFFCM disburses funds to award recipients, who then have 24 months to complete their project. Within three months of the completion of the project, the award recipient must submit a written report explaining in detail the use of the funds and the goals achieved in completing the project. Pt VI, Line 11b Form 990 is prepared by an outside tax professional. A copy of the form 990 is submitted to the Executive Director and to the Finance Committee for review and approval, then distributed to the full board for review. The Executive Director of the organization signs the form 990 before submitting to the U.S. Department of the Treasury. Pt VI, Line 19 Documents are made available to the public upon request.
