<Desc>In 2015, Trust for America's Health (TFAH) continued to work to improve the nation's commitment to an adequately resourced 21st Century prevention and public health system. Every project that TFAH undertook, from infectious diseases, to HIV/AIDS prevention, clean air, and childhood obesity, included specific recommendations for how to enhance, modernize, and hold accountable federal, state, and local public health departments. TFAH promoted public health policies that affect the entire nation, with an emphasis on how public health programs can provide protection and promote healthy communities regardless of geography.TFAH is committed to building healthier, more resilient communities to address the nation's racial, ethnic, geographic, and economic disparities. Further, a key goal is to build sustainable, scalable, changes that address common risk factors for chronic disease and behavioral health needs. TFAHs work towards improving the nation's public health capacity included non-partisan research and analysis, publication of science-based reports, outreach and educational activities, and the convening of subject matter experts about the need for a robust, effective public health system that is prepared to respond to a wide range of public health emergencies. At the core of TFAHs work is its efforts to develop and cultivate partnerships and build consensus. In this role, TFAH successfully convened public health experts on a variety of public health topics, including: opportunities for collaboration between health and education; identification of policies to integrate public health and community prevention into the reforming health care system; substance misuse; and the development of sustainable financing for population health.Among TFAHs achievements in 2015:Successfully helped make the case for the importance of the Centers for Medicare and Medicaid Services funding of a demonstration program promoting the linkage of health care and social services and addressing upstream social determinants to help improve health outcomes and lower health care costs. Launched a community prevention/multi-sector stakeholder webinar series in partnership with four sponsoring and 10 co-sponsoring organizations. The series focused on The Transforming Health System Bridging Public Health and Health Care and Community Prevention and Multi-Sector Engagement. Expanded TFAHs story bank with 45 new Prevention and Public Health Stories in the States, focused on topics ranging from obesity to community benefit to population health.
