<ExplanationTxt>Winter Haven Hospital and Winter Haven Womens Hospital took into account a variety of community-based organizations in the region during the CHNA process. Interviews were conducted with ten community leaders who encompassed a wide variety of professional backgrounds including 1) Public health expertise; 2) Professionals with access to community health related data; and 3) Representatives of underserved populations. The interviews offered community organizations an opportunity to provide feedback on the needs of the community, secondary data resources, and other information relevant to the study. Each interview was conducted by Tripp Umbach, a consulting firm representing the hospitals, and lasted approximately 60 minutes. All respondents were asked the same set of questions developed by Tripp Umbach and previously reviewed by a Winter Haven Hospital and Winter Haven Womens Hospital CHNA oversight committee. The purpose of these interviews was for stakeholders to identify health issues and concerns affecting residents in the communities served by Winter Haven Hospital and Winter Haven Womens Hospital, as well as ways to address those concerns. There was a diverse representation of community-based organizations and agencies among the 10 stakeholders interviewed. Those organizations represented included: -Angel Care Center Elouise -Peace River Center -Polk County Health Department -Polk Health Care plan -Winter Haven Hospital Emergency Department Physicians Group -Central Florida Health Care Inc. -Haley Center -Tri-County Human Services -Polk State College -Neighborhood Services Center In addition to the community -based organization interviews, Winter Haven Hospital and Winter Haven Womens Hospital gathered additional information by surveying vulnerable populations. Tripp Umbach worked closely with the CHNA oversight committee to ensure that community members, including under-represented residents, were included in the needs assessment through a survey process. A total of 216 surveys were collected in the Winter Haven Hospital and Winter Haven Womens Hospital service area which provides a +/- 6.66 confidence interval for a 95% confidence level. Tripp Umbach worked with the oversight committee to design a 32 question health status survey. The survey was offered in English, Spanish, and Vietnamese. The survey was administered by community- based organizations providing services to vulnerable populations in the hospital service area. Community based organizations were trained to administer the survey using hand-distribution. Surveys were administered onsite and securely mailed to Tripp Umbach for tabulation and analysis. Vulnerable populations were identified by the CHNA oversight committee and through stakeholder interviews. Vulnerable populations targeted by the surveys were residents that were: seniors, low-income (including families), uninsured, Latino, chronically ill, residents with a mental health history, homeless, residents with a literacy challenge, women of child bearing age, diabetic, and residents with special needs. The survey process lasted from August 2015 until September 2015. <FormAndLineReferenceDesc>Part V Section B Line 11</FormAndLineReferenceDesc>
<ExplanationTxt>Based on its most recent community health needs assessment (CHNA), Winter haven Hospital is committed to addressing the following significant community needs: 1) Improving access to health services 2) Decreasing the prevalence of clinical health issues 3) Improving healthy behavior and environments Progress was made to address each of the key health needs as prioritized within the 2015 Community Health Needs Assessment. Efforts were specifically dedicated to the following activities as noted below: Improve access to and coordination of mental health services o Active recruiting and retention strategies continue to maintain stability in Outpatient Prescriber staffing. Currently staffing pattern appears to meet the needs of those requesting access to services. Expansion to the indigent in the community will further improve access to care. o Working with Polk County government to establish a clinic within our behavioral health building to enhance the access to medical care for this underserved population. Continue advocacy outreach efforts to expand access o Navigators are in place across BayCare to facilitate enrollment in marketplace plans. Outreach activities continue (by navigators) to encourage enrollment in health insurance plans across various community locations. Team members are also available to interview uninsured patients in the hospitals and assist insured patients needing assistance for copays/deductibles (by phone or by appointment). The number of people served continues to increase. Implement Health Ministry program o 8 RN/church pairs with stated intent or strong interest in participation. o Continue to build relationships and address barriers to participation. o Proposed 8-wk Church Health Promotor curriculum program to be coordinated by hospital-based FCN. o CHP program also useful to FCN Partners in building a churchs Health Ministry Team. Improve access to Mental Health treatment in the community o WHH-CBH Outpatient Services continues to conduct a suicide screening with all new clients at intake and whenever there is a deterioration of status. The PHQ-9 is also used to determine severity of depression which depending on the score initiates a referral to a Prescriber. Participation at several health fairs throughout the year has served as a method of prevention to ensure individuals have information about where to access services. Many mental health provider have become certified in Mental health First Aid to improve their acuity at identifying and treating those at risk for suicide. o Suicide Risk Modified to incorporation within the Cerner Medical record for behavioral documentation (June 2015). Implement plans to collaborate with community-based programs addressing needs of expecting mothers at risk for poor birth outcomes o Continued relationship with Healthy Start Coalition to promote and support healthy pregnancies, babies, and families. o Maintained and started new quality projects with Florida Perinatal Quality Collaborative. o Continue partnership with Polk County Health Department and Central Florida Health Care by providing delivery services for patients that receive prenatal care through their clinics. o Continue to join efforts with CFHC to recruit area physicians to support high risk patients. Improve care coordination and outcomes for heart failure (CHF) patients o Heart Function clinic opened in November with 6 patients seen in November and 18 patients seen in December. o Participation on system wide collaborative. o Faith community nurse developing network Develop and implement plans to meet the health needs available to undocumented and uninsured residents o Political Action Committee Developed proceeding with referendum planned for November 2016 general election. o County board approved November 2016 ballot referendum Increase the availability of treatment in Polk County for individuals with co-occurring mental health and substance abuse o TCHS Co-location being pursued. All those with co-occurring mental health and substance abuse are served and referrals to appropriate substance abuse providers are made as well. Continue to offer education and community support o We continue to offer free health education seminars and screening test. o We have initiated a policy to discourage early elective deliveries which has successfully reduced early elective deliveries. o We continue to be a smoke free campus. o Provide various support groups, educational seminars, and health fairs. o We sponsor AHA and ACS. o We have implemented a faith community nurse program. o We participate in the 'Building a Healthier Polk' initiative to reduce obesity. o We co-sponsored the Polk County community needs assessment forum. While it is our desire to have a maximum positive impact on our community all of our goals were not achieved: Substance Abuse We have been challenged to make a significant impact on substance abuse. The rise of new substances such as bath slats and K2 has provided additional challenges to our services for mental health as well as emergency care. We will continue to work with organizations to establish better networks with those organizations that specialize in the treatment of these patients. Expanding availability of Primary Care Physicians While not specifically identified as a goal in our CHNA we have focused on the availability of primary care physicians to enhance access to care in our community. In 2015 we experienced significant turnover in our employed primary care physicians resulting in fewer access points for our services. Our original assessment of Graduate Medical Education as a means to address this concern in 2014 was deemed unfeasible at that time. We are currently participating in a broader group assessment of opportunities across the community to implement a graduate medical education program in our community for the purpose of increasing access to primary care through the program and develop a feeder system to retain primary care physicians within our community upon completion of the program.</ExplanationTxt>

<ExplanationTxt>LINE 3 Patients who are uninsured or underinsured and cannot pay for hospital services are eligible for charity consideration. These patients are screened by designated team members in our Financial Assistance Department. The Agency for Health Care Administration (AHCA) defines charity eligibility at 200 percent of the federal poverty guidelines, unless the total hospital bill is more than 25 percent of the patient's annual income. Medicaid recipients who have exceeded their coverage limits are also considered for charity care. Winter Haven Hospital goes above and beyond the AHCA requirements by providing additional "hardship" charity for patients who are at 250 percent of the federal poverty guidelines. In addition, an uninsured discount of 40% is given to any patient who does not have insurance coverage or benefits. There is no income or asset test required for the uninsured discount. Patients receive an additional 10% discount if the account is paid within 30 days. part i, Line 7 Financial assistance and means-tested government programs costs (lines A through D) are determined using our cost accounting system, which captures all inpatients and outpatients, including emergency room patients. The system also captures all patient pay types - private insurance, Medicare, Medicaid, uninsured and self-pay. The costs have been offset by any payments received from Medicaid or any other uncompensated care program. Other benefits at cost (lines E through J, as well as amounts reported in Part II) were compiled by the community health department using the Catholic Health Association guide for planning and reporting community benefits Part I, Line 7 Col (f): There is $31,642,085 (accrual basis) of bad debt expense which is included in Form 990, Part IX, Line 25. <ExplanationTxt>The Hospital helps build the community in two major ways. First, if the hospital finds that that there is a shortage in a certain health care specialty, it works to recruit those types of physicians to the area. In the past, the hospital has commissioned a manpower study to determine what the community's needs are. The second major way the hospital helps the community is through subsidizing the cost of anesthesia for women giving birth at the Regency Center for Women and Infants. This benefits the community by allowing those who are less fortunate to have access to anesthesia services.</ExplanationTxt>

<ExplanationTxt>Lines 2 & 3 Bad debt expense is reported as total bad debt for the facility. The amount of bad debt expense attributable to patients eligible for financial assistance is calculated as a charge ratio, derived from data sampling. The resulting charge ratio is then applied to total bad debt accounts of the organization, which calculates the bad debt attributable to finance assistance. The state of Florida requires the patient to provide certain documentation in order to qualify for financial assistance. In cases where the patient has not responded to hospital requests or billing statement alerts, those accounts are processed as bad debt, if unpaid. <ExplanationTxt>Cost reports were used to report Medicare allowable costs. Medicare defines allowable costs as those appropriate and helpful in developing and maintaining the operation of patient care facilities and activities. It specifically excludes certain costs that are not directly related to patient care. The hospital incurs additional expense related to the provision of care to Medicare patients that Medicare has deemed non-allowable. This additional expense includes costs of physician services (emergency on-call fees, hospitalist program, recruitment, etc.), advertising costs, cafeteria costs for meals sold to visitors, etc. The hospital attempts to collect coinsurance and deductibles from Medicare beneficiaries. To the extent collection efforts are unsuccessful, Medicare reimburses the hospital at 70% of unpaid amounts. The following table reconciles the surplus or shortfall from Line 7 to the actual surplus or shortfall. The additional costs were allocated to Medicare based upon Medicare's percentage of total allowable costs. The unpaid coinsurance/deductibles were estimated using historical collection results. Any shortfall amounts have not been treated as community benefit. -Line 7 Surplus or (Shortfall) ($13,428,891) -Additional non-allowable costs and unpaid/non-reimbursed coinsurance/deductibles ($5,697,223) -Total Surplus or (Shortfall) ($19,126,114)</ExplanationTxt>

<ExplanationTxt>Part VI, Line 15a & 15b - Process used for compensation review and approval. The organization uses an independent compensation committee, appointed by the Board of Directors. The Compensation Committee's purpose is to provide oversight for the organization's executive compensation program, review and approve compensation and benefits for all "disqualified persons" subject to the Intermediate Sanctions regulations issued under Section 4958 of the Internal Revenue Code (including the Chief Executive Officer, Chief Operating Officer & Chief Financial Officer, other system and entity executives, and other disqualified persons as defined in the Intermediate Sanctions regulations (i.e., voting members of the governing body, family members, former officers)), and establish the compensation philosophy for all other executives. This committee engages nationally recognized compensation consultants to assist them in review of executive compensation. The compensation consultants provide a review of each vice president and above in the system to determine if that employee's compensation is reasonable when compared against market standards. The data reviewed comes from compensation studies that include comparable compensation for similarly qualified persons in functionally comparable positions at similarly situated organizations. The organization keeps contemporaneous minutes of the compensation committees meetings and decisions. External consultants review compensation every other year with the last review occurring in November 2015 but the compensation committee regularly monitors compensation and all other procedures are followed annually. Part VI, Line 19 - Avail of Gov Docs, Conflict of Interest Policy, & Fin Stmts to Gen Public Winter Haven Hospital's governing documents are available to the public via Sunbiz.org.</ExplanationTxt>
