Within the Masonicare community there were two stations set up in plain sight of visitors. Blank questionnaires available and a blue lock box in which they could return complete questionnaires. The two locations were at the main entrance of the Masonicare Health Center (MHC) and the Medical Office Building waiting room. The boxes were checked regularly. Additionally, Masonicare volunteers were asked to fill out a questionnaire and return them to the volunteer coordinator.The intercept technique was also employed to collect responses. A representative would stand in the vicinity of the main entrance for MHC and ask passing residents or visitors to take a few moments and fill out the survey. Another method used to collect responses was through the online survey tool, Survey Monkey. At the physical collection stations we left slips of paper that provided the survey website. If people were in a rush and could not fill out a hard copy, we asked that they access the site and complete the questionnaire at their convenience.The final way responses were collected was by a representative traveling to the Senior Centers within the community served by MHC, Wallingford Senior Center and Meriden Senior Center. Once again, the intercept technique was used. All physical responses were collected and manually entered into the Survey Monkey database. Any surveys that were illegible or filled out incorrectly were not used. The only piece of information used from incorrectly filled out surveys was the answers to the demographic questions and the additional comments if any were given. If there were no additional comments the questionnaire was completely disregarded; no demographic information or any other information was used. Part V, Line 5 - Account Input from Persons Who Represent the Community Masonicare's Community Health Needs assessment responses were collected over a three month period, January 1st - March 30th 2014. During this time 175 questionnaires were collected and entered into a database for analysis. The result did not determine any novel needs but instead affirmed the need for continuation and augmentation of our current programs. The top ranked needs identified were:(1)Access to wellness, disease prevention, and ongoing healthcare services(2)Affordable housing(3)Financial difficulties in paying for everyday needs(4)Home care for those with limitation in their daily activities of living(5)Difficulty understanding Medicare/Medicaid Part V, Line 7d - Description of Making Needs Assessment Widely Available Subsequent to September 30, 2015 the CHNA report has been made widely available to the public on Masonicares website. Part V, Line 8 - CHNA implementation strategyAlthough the CHNA report and implementation strategy was reviewed and discussed at the Board level, formal Board-level approval of an implementation strategy did not occur. Part V, Line 15e - Other Method For Applying For Financial Assistance Billing statements and follow-up correspondence note the availability of Financial Assistance Programs as one of the available payment options. Part V, Line 16i - Other Means Hospital Facility Publicized the Policy Subsequent to September 30, 2015 the Financial Assistance Policy and related application form have been made available on Masonicares website www.masonicare.org (Our Services, Hospital Care for Seniors, Financial Aid Requirements and Applications).Also subsequent to September 30, 2015 the Financial Assistance Policy and related application form are provided at all hospital registration locations. Part V, Line 22d - Other Billing Determination of Individuals Without Insurance MHC does not provide emergency care. For other medically necessary care, the maximum amounts that can be charged to FAP eligible individuals is less than the published rate. Part I, Line 7 - Explanation of Costing Methodology Masonicare Health Center (MHC) provides a variety of geriatric services to the Wallingford, CT community. Approximately 70% of MHCs revenues come from the provision of skilled nursing facility services (375 licensed beds); approximately 10% of revenues come from residential care home (86 units), independent living apartments (93 units) and other sources; and approximately 20% of revenues come from the provision of geriatric hospital services (geriatric medical psychiatric inpatient services 29 beds licensed and available; geriatric acute care inpatient hospital medical services exclusive of emergency services and surgical services 62 beds licensed with 30 beds available; and certain geriatric outpatient services). Based upon the geriatric population receiving hospital services, less than 2% of hospital revenues come from private pay / self pay sources; this represents less than 0.5% of MHC total revenues.The amounts reported in this section represent amounts attributable to hospital services only. Part III, Line 2 - Methodology Used To Estimate Bad Debt Expense Masonicare Health Center (MHC) calculates the estimated allowance for doubtful accounts by applying appropriate reserve factors to the various aging categories of patient accounts receivable. The provision for bad debts is the amount recorded to have the allowance for doubtful accounts stated at the calculated value. Part III, Line 4 - Bad Debt Expense The description of the organizations Provision for Bad Debts can be found on page 12 of Masonicares consolidated financial statements for the year ended September 30, 2015. Part III, Line 8 - Explanation Of Shortfall As Community Benefit None of the Medicare shortfall was treated as community benefit. The source of the costing methodology was the Medicare cost report Part III, Line 9b - Provisions On Collection Practices For Qualified Patients Masonicare Health Center (MHC) provides financial assistance to patients who meet certain criteria under the financial assistance policy without charge, or at amounts less than established rates. While MCHs overall collection policy allows for the use of extraordinary collection actions, such extraordinary collection actions are not authorized for use in the collection of bills related to hospital services. Part VI - Community Information Masonicare Health Center (MHC) offers a wide variety of geriatric services to the Wallingford, Connecticut community that include:a. 375 skilled nursing and rehabilitation bedsb. 93 independent living apartmentsc. 86 residential care apartmentsd. 29 acute geriatric psychiatric hospital bedse. 30 acute medical hospital bedsf. Outpatient speciality clinics MHC is licensed as a chronic disease hospital and as such does not have emergency room or provide any emergency medical services. MHC also does not provide any surgical services. The revenues for the hospital portion of the business represents approximately 20% of the program service revenues of MHC.MHC, located in Wallingford CT, services the greater Wallingford area. MHC is centrally located in the state providing easy access to the surrounding towns. In addition to the inpatient acute geriatric medical and inpatient acute geriatric psychiatric services, its outpatient specialty clinics provide easy access and high quality services to the Town of Wallingford and the surrounding towns.Masonicare, the parent company of Masonicare Health Center (MHC), serves the entire state of Connecticut through its various affiliates. As a non-profit 501(c)(3) organization, Masonicare is proud to provide free education programs and services to individuals and families throughout Connecticut. For the year ended September 30, 2015 Masonicare and all of its affiliates across our continuum donated 3,008 hours of their time to benefit the community through home visits and educational programs. These donated hours and services had a monetary value of $77,218 and touched 8,679 members of the community.
