Food Shelf distributed 152,602 lbs of food, 2,936 appointments in which 661 unique members accessed the foodshelf. Highlights:added additional evening hours of service, created drop-in appointments which resulted in a significant decrease in the number of emergency appointments. Nutrition:-22% increase in number of appointments. 727 medical nutrition sessions to 221 individual members, 188 referrals to the state HIV/AIDS Health Program for nutritional therapies, 95 referrals to Moms Meals, a meal delivery option for Greater Minnesota people living with HIV. Highlights:began providing body composition measurement and health counseling, started a community garden and associated walking program. 427080 Member Services: Includes a medical case management program which served 119 members and a non-medical case management program which served 45 members. Highlights:Department restructured to provide ease of transition from care linkage to case management: non-medical program is being newly developed and saw huge growth in six months. 264413 Outreach and Education: Includes a health and wellness program that offers educational seminars, outreach services, HIV testing and a resource library with Internet access. The Outreach/Care Linkage program had 246 health education encounters, 6,386 outreach encounters, over 50% were in the Afican born community through our newly developed early intervention program, 34 care coordination sessions, 448 HIV tests to high-risk individuals, 13,222 condoms and safer sex materials distributed and 21 linkages of persons living with HIV who were out of medical care.
