Conflict of Interest Policy Monitoring & Enforcement Form 990, Part VI, Line 12c An annual questionnaire and disclosure form was developed by our external legal counsel. Every July 1 the questionnaire is sent out to all officers, trustees and key employees. They are checked to make sure that they are all returned and signed. They are then reviewed by the CEO and CFO.
