Part X, Line 2: The Foundation has been determined to be exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code and will be taxed only to the extent it has taxable trade or business income unrelated to its exempt purpose. At June 30, 2017, fiscal years ended June 30, 2014, 2015, 2016 and 2017 remained open to examination by federal and state taxing authorities. Form 990, Part VI, Section B, line 11b Form 990 is reviewed by the full Board of Directors and the Sigatory Officer prior to signing. Form 990, Part VI, Section B, line 12c The organization regularly and consistently monitors and enforces compliance with the conflict of interest policy. Any board member having knowledge of any violation of this policy shall promptly report such violation to the Board Chairman or the Chief Executive Officer.
