The following is more information on the community wellness programs:The community wellness programs offer education, support, and linkages to isolated mothers, overwhelmed and frustrated parents, impressionable children, and disadvantaged adults through four initiatives offered free of charge in both English and Spanish: - Family Support Services: Promotes strong, healthy, parent-child relationships, as well as the social, emotional, and intellectual development of young children. - Community Nursing Services: Provides disease prevention and health promotion services that empower and encourage clients to become active participants of their own health and improve their quality of life. - Mental Health Services: Offers culturally sensitive counseling services and emotional support to adults who have limited or no access to health insurance. Help is available for those experiencing depression, anxiety, the effects of trauma or who are going through a difficult time or life transition. - Access to Healthcare: Links families, individuals and seniors to health insurance, public benefits, community resources and senior wellness programs.The Community Wellness Program also supports pregnant moms by connecting them to newly enhanced prenatal education classes at Saint Anthony Hospital. In FY16, the Community Wellness Program worked with the hospital's prenatal services department and OB patient navigators to support pregnant women. This is an additional service for our patients and their families in the continuum of care.The Community Wellness Program has a history of developing community-defined social services that fill in gaps in the community as well as fostering strong relationships with community organizations in order to impact the social needs of underserved communities.
