Conflict of interest policy compliance Part VI line 12c Board members receive this information at board orientation and also sign an annual commitment of service which addresses conflicts of interest. The personnel policies address conflicts of interest for staff members and these are provided to the staff members during orientation when hired. Personnel policies are reviewed annually normally in July. CEO executive director top management comp Part VI line 15a Data is compiled related to the President/CEOs current salary and benefits, years of service, responsibilities, and accomplishments. This data along with comparability data from other local agencies is used by the executive committee to determine the compensation package for the President/CEO. The executive committee of the board of directors conducts an annual performance review with the President/CEO and all relevant data is properly reviewed, approved, and documented. Other officer or key employee compensation Part VI line 15b During the fall of 2015, the Day Nursery board completed a comparability review of compensation for all employees including the management positions. Form 990 availability to public Part VI line 18 Requests for the Form 990 may be submitted in writing to the President/CEO. Governing documents etc available to public Part VI line 19 Requests for governing documents, conflict of interest policies, and financial statements may be submitted in writing to the President/CEO. Explanation of other changes in net assets or fund balances Part XI line 9 A prior period adjustment was recorded in the financial statements for the year ended December 31, 2015. This prior period adjustment was due to a trust received by the Day Nursery of Abilene in late 2015, but was not recorded in 2015. After further review of this trust in 2016, it was determined that this trust met the accounting requirements to be recorded as a contribution. This prior period adjustment increased contribution income $229,604 and decreased investment income by $1,645.
