The qualifications for the honorary board member was amended. Form 990, Part VI, Section B, line 11 The Assistant Director of Accounting reviews the Form 990 and submits it to the Director of Accounting who then submits to CFO for review. When the CFO approves the Form 990 it is ready for submission. Form 990, Part VI, Section B, line 12c All directors, officers, and members of senior management shall complete an annual conflict of interest survey. All directors, officers, and members of senior management are required to disclose any relationships they have with YKHC's vendors. The Compliance department will be responsible for developing and administering the survey and the Operational Compliance Committee will review the survey results and decide whether a conflict exists. The Legal Department is also available to consult with directors, officers, and members of senior management on conflicts of interest. SLT and the Board receives a presentation of the audit results from the auditing partner. The selection of the independent accountant goes through a request for proposal which is then scored and choosen accordingly.
