Form 990, Part VI, Section A, Line 3 The school contracted with a management company, Accel Schools LLC, to provide certain instructional, administrative and financial services. Form 990, Part VI, Section B, Line 11b The Form 990 is prepared and reviewed by the Fiscal Officer. The return is then provided to the Governing Board along with a summary of key items in the return. Once approved, the return is signed and filed by the fiscal officer. Governing Board members and key personnel are expected to disclose any relationships that could potentially be perceived as a conflict of interest.
