These patients are screened by designated team members in our Financial Assistance Department. The Agency for Health Care Administration (AHCA) defines charity eligibility at 200 percent of the federal poverty guidelines, unless the total hospital bill is more than 25 percent of the patient's annual income. Medicaid recipients who have exceeded their coverage limits are also considered for charity care. Winter Haven Hospital goes above and beyond the AHCA requirements by providing additional "hardship" charity for patients who are at 250 percent of the federal poverty guidelines. In addition, an uninsured discount of 40% is given to any patient who does not have insurance coverage or benefits. There is no income or asset test required for the uninsured discount. Patients receive an additional 10% discount if the account is paid within 30 days. Part I, Line 7 Financial assistance and means-tested government programs costs (lines A through D) are determined using our cost accounting system, which captures all inpatients and outpatients, including emergency room patients. The system also captures all patient pay types - private insurance, Medicare, Medicaid, uninsured and self-pay. The costs have been offset by any payments received from Medicaid or any other uncompensated care program.Other benefits at cost (lines E through J, as well as amounts reported in Part II) were compiled by the community health department using the Catholic Health Association guide for planning and reporting community benefits Part I, Line 7 Col (f) There is $8,330,837 (accrual basis) of bad debt expense which is included in Form 990, Part IX, Line 25. This amount was removed in the calculation of the Percent of Total Expense on Schedule H, Part I, Column 7f. Part III, Lines 2 & 3 Bad debt expense is reported as total bad debt for the facility. The amount of bad debt expense attributable to patients eligible for financial assistance is calculated as a charge ratio, derived from data sampling. The resulting charge ratio is then applied to total bad debt accounts of the organization, which calculates the bad debt attributable to finance assistance. The state of Florida requires the patient to provide certain documentation in order to qualify for financial assistance. In cases where the patient has not responded to hospital requests or billing statement alerts, those accounts are processed as bad debt, if unpaid. Part III, Line 4 The organization's financial statements include a footnote that describes bad debt expense on page 10 of the BayCare Health System, Inc. and Affiliates Notes to Consolidated Financial Statements. THE HOSPITAL INCURS ADDITIONAL EXPENSE RELATED TO THE PROVISION OF CARE TO MEDICARE PATIENTS THAT MEDICARE HAS DEEMED NON-ALLOWABLE. THIS ADDITIONAL EXPENSE INCLUDES COSTS OF PHYSICIAN SERVICES (EMERGENCY ON-CALL FEES, HOSPITALIST PROGRAM, RECRUITMENT, ETC.), ADVERTISING COSTS, CAFETERIA COSTS FOR MEALS SOLD TO VISITORS, ETC. THE HOSPITAL ATTEMPTS TO COLLECT COINSURANCE AND DEDUCTIBLES FROM MEDICARE BENEFICIARIES. TO THE EXTENT COLLECTION EFFORTS ARE UNSUCCESSFUL, MEDICARE REIMBURSES THE HOSPITAL AT 70% OF UNPAID AMOUNTS. ANY SHORTFALL AMOUNTS HAVE NOT BEEN TREATED AS COMMUNITY BENEFIT. - LINE 7 SURPLUS OR (SHORTFALL) ($3,150,815) - ADDITIONAL NON-ALLOWABLE COSTS AND UNPAID/NON-REIMBURSED COINSURANCE/DEDUCTIBLES: ($439,146) - TOTAL SURPLUS OR (SHORTFALL): (3,589,961) Part III, Line 9b Patients who are unable to pay are encouraged by BayCare Health System representatives, via personal interviews, signage, on patient billing statements, brochures or Customer Service phone calls, to submit financial information to the Financial Assistance Department to determine eligibility for programs, such as County, Medicaid, Disability, Victims of Crime, Charity, etc. For those patients who provide all the necessary documentation and qualify for charity according to the Financial Assistance policy, (defined in Part I, line 3c), patients' account balance would be written off completely to charity and not billed to the patients. Part VI, Line 2 The hospital assesses the health care needs of the community by participating in several community organizations that develop community strategic plans. The Hospital also works with the Polk County Health Department in order to conduct studies and help set priorities. Additionally, the Hospital works with and utilizes several community health surveys conducted by Polk Healthcare Alliance. Part VI, Line 3 Bartow Regional Medical Center's Financial Assistance team members are dedicated to assisting patients in obtaining assistance through federal, state and local government programs or through the Bartow Regional Medical Center, Inc financial assistance policy. Signage and brochures are available, as well as team members whose full responsibility is to assist patients in the emergency room and on inpatient units. The Financial Assistance team interviews patients for all available programs, assists the patients in completing applications to government agencies and for hospital charity care, advises patients regarding available community resources for health care, reviews and approves patient requests for charity care, and provides education and support to the patient throughout the assistance process. In addition to the aforementioned comprehensive process, Bartow Regional Medical Center, Inc also informs and educates patients who may be billed for patient care, but may be eligible for charity or other programs, via patient billing statements and customer service representative calls. The goal in using these various means is to effectively communicate with the entire patient population so they are informed and educated about their eligibility for assistance. Part VI, Line 4 Bartow Regional Medical Center is in a suburban setting serving parts of Polk and Hardee Counties. The average income falls below the state average and the national average. The population served is predominantly Caucasian and high-school or higher educated. Hispanics are the second largest ethnic group representing 18.3% of the population. 13.5% of households have annual household income below $15,000 per year. The population served by Bartow Regional Medical Center is expected to grow 5.9% in the next 5 years. This is higher than the expected growth rate of 3.8% for the United States. The fastest growing population segment is Age 65+. Bartow Regional Medical Center is part of BayCare Health System that serves west central Florida. The area served by Bartow Regional has 4 hospitals, Lakeland Regional Health System is the only non-BayCare facility in the service area. There is 1 federally designated underserved area in Bartow Regionals service area. With the over 65 population expected to grow 15.5% in the next five years, the health care needs of the service area are expanding and changing. Based on Florida inpatient discharge data for the period of 10/01/2015-09/30/2016, the payer mix for the geographic area consists of 49.8% Medicare/Medicare HMO, 18.7% Medicaid/Medicaid HMO, 20.2% Commercial Insurance, 7.1% Self pay/Non-pay, and 4.2% Other. Part VI, Line 5 Bartow Regional Medical Center (BRMC), located in Bartow, Florida and part of the BayCare Health System, is a 72-bed acute care hospital serving south Lakeland, Bartow, Ft. Meade, Mulberry, rural south Polk County and northern Hardee County. Approximately 150 physicians and 300 healthcare professionals at Bartow Regional Medical Center provide a variety of services including, inpatient and outpatient surgery, imaging, emergency medicine, wound care and cardiac care. The hospital is equipped with four surgery suites and two gastroenterology suites. Community Outreach and Partnerships Community outreach and partnerships are an essential expression of Bartow Regionals commitment to improving the health of communities we serve. Through education and screenings, community connections, and free or reduced-cost clinical care to the uninsured or underinsured, we continue to contribute to the wellbeing of Bartow and surrounding communities. Screenings, Support and Education During heart disease awareness month, Bartow Regional hosts free screenings for participants to learn more about their heart health. The screenings include blood pressure, body mass index and total cholesterol readings to provide a snapshot of overall health and recommendations for the next step toward living a healthier life. Also provided were prescreening for diabetes and blood glucose, which if risk was detected, then an A1C was offered. Forty-two community residents participated in the 2016 events. Health Care Navigators support all BayCare facilities to assist consumers to shop for affordable health insurance in the Health Insurance Marketplace. Navigators are available to patients, family members, team members and families, and others in the community needing assistance with applying, determining eligibility for coverage and for exemptions, filing an appeal or to simply learn more about the Affordable Care Act and the Health Insurance Marketplace. One-on-one guidance is available by appointment and throughout the year at enrollment events and other community activities. At BRMC, our Chaplaincy Services provide spiritual and emotional support to patients and their families of all religious traditions and faith communities as well as those with no faith background. In addition to providing a Chapel for prayer and quiet reflection, the staff chaplain participates in family conferences regarding patient care; offers ethics consultation services; assists with the explanation and distribution of Advance Directives; provides new patient introductions to their services; and welcomes and assists local clergy as they visit their members. Support and education is also provided in the form of support groups. In 2016, Bartow Regional facilitated support groups for diabetes management and cardiac health. Health education lectures by physicians and other health care providers are held at Bartow Regional facilities on a variety of topics. Lecture topics in 2016 included bariatric surgery, healthy eating, and total joint replacement. Community Connections In 2016 as part of Bartow Regional Medical Centers transition to a community hospital managed by BayCare, a Board of Trustees was established to comprise of a diverse set of community members who believe in the mission, values, and vision of the hospital and who donate their time, talents and support. The primary responsibility of the Board is to assist with policy formulation, decision making and oversight by ensuring decision and actions conform to Bartow Regionals strategic plans and budgets and produce intended results. Leaders from various industry backgrounds and education represent the board, including county government, health care foundation, legal, finance, and banking. The Hospital supports a full-time physician recruiter and actively recruits physicians in primary care and specialty care areas where it has identified a community need. The hospital also works closely with physician groups to assist them in recruiting new physicians to our area. The Hospital enables its leaders to participate in organizations that benefit the well-being of the community including Polk County Health Plan Citizens Healthcare Oversight Committee, Bartow Economic Development Council, and the Bartow Chamber of Commerce. Bartow Regional and its employees provided financial support, in the form of sponsorships and fund raising, for not-for-profit organizations whose missions are to improve the health and well-being of our community. In 2016, we donated to community organizations including American Heart Association, Bartow Community Healthcare Foundation, Kiwanis Club of Bartow, Citizens CPR, and Polk County Fire Rescue. Charitable Clinical Care Polk HealthCare Plan is a public/private partnership that allows for a comprehensive managed health care program for eligible Polk County residents. Bartow Regional participates in and accepts the Polk Healthcare Plan to help increases health care services to low income residents of Polk County. The Plan provides an integrated system of health care and social services through a wide range of primary and preventive medical care, specialty medical care, hospitalization, diagnostic testing, ancillary services and pharmaceuticals. In essence, the Plan functions as a County-operated HMO for qualified Polk County residents. Health Professionals Education Bartow Regional assisted nearly 180 students of health-related fields to fulfill their academic goals and requirements through internships with hospital departments in 201. Schools that partnered with us include the University of South Florida, University of Florida, Polk State College, Keiser University and Galen University. The health-related fields sought and hospital departments providing assistance were: nursing, dietary, pharmacy, physical therapy, radiography, respiratory, sonography, and surgical technology. Part VI, Line 6 Bartow Regional Medical Center is part of BayCare Health System, Inc.
