Each funding source conducts a minimum of one monitoring visit during the contract year. Once members have had time to review the form, JobWorks' president and VP of Finance will meet with the board to discuss the Form 990 and answer any questions. Upon approval of the board of directors, JobWorks' president will submit Form 990. Form 990, Part VI, Line 12c: Explanation of Monitoring and Enforcement of Conflicts JobWorks requires that each board member and each member of JobWorks' management team sign a conflict of interest statement on an annual basis.
