Grantmaker's Description of How Grants are Used The Regional Center provides case management services, including quarterly visits to consumers residing in residential care facilities or supported living. In addition, persons who live independently in the community are visited quarterly. Persons living with family members are seen annually. Individual program plans are designed for each consumer, and the case manager and supervisor monitor progress against objectives through observation, inquiry of third party providers (service providers), family members and written reports from the service providers. The plans and progress against objectives are discussed in the visits with the consumer and families. Service providers may be asked to attend the visits. The Regional Center's fiscal division also audits a sample of third party vendors and its community service division also performs quality reviews of third party vendor services.
