Highlights: a healthy foods policy means the food shelf and meal program are procuring and sourcing ingredients with maximum nutrition benefits for our members. The registered dietician worked with 244 members to help them work toward nutrition-related goals. 82 tons of groceries were dispensed from the food shelf. 477484 Member Services: Includes a medical case management program which served 154 members and a non-medical case management program which served 65 members. Highlights: case management linked 229 members to resources. Member services was expanded to include regular activities in the space including trivia, bingo, community conversations. Aliveness also had a year-long residency with Patrick's Cabaret, which offered monthly writing workshops to members. Member services held a total of 149 events for members. 225678 Outreach and Education: Includes a health and wellness program that offers educational seminars, outreach services, HIV testing, and a resource library with internet access. Highlights: outreach and care linkage continutes to grow and focus on at-risk populations including Afrian born and Latino communities. 367 HIV tests were administered to help people know their HIV status and 17 people out of care were linked to medical care. A partnership with Valhalla Place is getting Naloxone (an opiod overdose reversal kit) into the community and 5 lives have been saved as a result of distributing these kits.
