Form 990, Part VI, Section B, line 11b The Executive Director and the Finance Manager review the 990 form prior to filing and Family Focus works with an outside accounting firm to ensure accuracy. Form 990, Part VI, Section B, line 12c If a member finds she/he is in a real or perceived conflict of interest at any time during her/his tenure, including prior to a meeting, then she/he: (a) will immediately make a full disclosure to the Board; (b) will withdraw from the decision-making process regarding the conflict of interest; and (c) will not attempt to influence other decision-makers regarding any matter affected by the conflict of interest. If a member find taht she/he is in a real or perceived conflict of interest during a meeting, then she/he: (a) will immediately verbally disclose the conflict; and (b) will follow the procedures listed in (a-c). if any individual or organization perceives that a member is in real or potential conflict, she/he/they may bring the matter to the Board's attention in written form. Subsequently, the Board will strike an inquity committee of 3 consisting of at least one Board member (normally the President), the Executive Director, and a third-party volunteer who is agreeable to both the complainant and the individual whose conduct is under scrutiny; if the President or Executive Director is under scrutiny, the Board will appoint a suitable replacement (normally another Board member).
