Southwest Medical Center Part V, Section B, Line 11: Emergency RoomSWMC has campaigned and given documentation to the community to educate patients about when they should go to their primary care clinic and when they should go to the ER.Access to Primary Care and Primary Care SpecialistsSWMC continued to collaborate with regional primary care providers and share best practice delivery methods in order to increase access to primary care. SWMC has also worked to define expanded orientation programs for prospective new providers and improve new provider retention. SWMC has strengthen relationships with Wichita State University, the University of Kansas, and Rocky Vista by offering medical student rotations and helping recruit medical professionals to the area. SWMC continued recruiting efforts for providers. However, SWMC did not have much success in hiring due to physician shortages around the country. SWMC used the University of Kansas for recruiting in 2024.Mental Health: Diagnosis, Placement, Aftercare, Provider, and Treatment SWMC worked with staff and community education and met with KHA and other educational stakeholders regarding the shortage of mental health providers. SWMC added depression screening questions on intake in both the clinics and ER. The Medical Center did not have much progress in providing mental health service "first aid" training for law enforcement, schools, and other first responders in the community or collaborating with community elementary, middle, and high schools to educate students on mental health because of other more pressing issues.Childcare (Availability / Cost)SWMC representatives attended economic development council meetings in order to discuss the shortage of licensed child care providers in the area. SWMC staff continued showing educational videos to parents before discharging the baby in order to provide child safety and first aid training to parents. SWMC continued providing materials to educate young families on childcare and community resources. Collaboration with other CAH'sSWMC is continuing contacting regional hospitals and health care providers to promote and establish collaboration among these stakeholders. SWMC has partnered with KHA to help promote and develop healthcare initiatives that benefit all healthcare providers in Kansas. SWMC CEO has established a quarterly meeting with area hospital CEOs to dicuss issues, collaboration, and ways to help each other.Sex Health / STD'sSWMC SANE/SART program is still in service. This program is discussed with many of the women's groups in the community. SWMC provides documentation informing patients about steps they can take to prevent STDs.Access to Healthcare SpecialistsSWMC receive constant opinions and ideas from providers and community members regarding their thoughts on medical needs in the community. Telehealth has become a primary function of care with COVID-19. We also have telehealth related to stroke and neurology. SWMC continued to work with area medical schools to strengthen medical student placement relations and focus on the placement needs of students. Student placement has increased since last year. SWMC has begun letting students shadow in some areas of the Hospital. SWMC continued recruiting efforts for providers. SWMC began contracting with Sound Anesthesia for management of CRNA services. SWMC ended its contract with the previous recruiter and began a new contract with the University of Kansas for recruiting.SWMC attempted to ask local dentists see Medicaid patients one day a month as a community service. However, most local dentists do not take Medicaid and are not willing at this time to do the service one day a month. SWMC did partner with First Dental to staff a booth at the 2024 health fair in order to provide free screenings and hand out informational materials regarding dental care. The cost to charge ratios used include the overall cost to charge ratio derived from Worksheet 2 as well as individual department cost to charge ratios identified for the calculation of subsidized health services information. Part I, Line 7g: Subsidized Health Services are made up of services that the Medical Center provides despite a financial loss to the organization. These services include physician clinics. Of the $17,115,082 in total costs associated with subsidized health services, $15,195,934 were due to physician clinics. Part III, Line 4: The Medical Center provides for accounts receivable that could become uncollectible in the future by establishing an allowance to reduce the carrying value of such receivables to their estimated net realizable value. The Medical Center estimates this allowance based on the aging of its accounts receivable and its historical collection experience for each type of payor.Part III, Line 2Provisions for bad debts are related to accounts that have been deemed uncollectible during the year. The uncollectible amount is then written off pursuant with Medical Center policy. The cost of the bad debts reported was determined using the cost to charge ratio and total bad debt expense.Part III, Line 3The estimate of provisions for bad debts attributable to patients eligible under the organization's financial assistance policy was determined using the federal poverty rate multiplied by the Medical Center's bad debt expense and cost to charge ratio. It is our belief that the portion of the Medical Center's bad debt expense attributable to patients eligible under the Medical Center's financial assistance policy should be included as community benefit. As a not-for-profit, patient care is provided to all, regardless of ability to pay for that care. Making quality patient care available to all in our community, regardless of their economic means, qualifies bad debts as a community benefit. Part III, Line 8: Amounts were obtained from the cost report filed with Medicare for the period ending December 31, 2024. The Medicare shortfall should be considered a community benefit because our mission is to promote health in the community and we do not limit the care available to any patients, including those covered by Medicare. We are relieving a government burden by providing care to Medicare patients. Tax-exempt hospitals are expected to participate in the Medicare program. Part III, Line 9b: The Medical Center's collection policy does not contain a provision on the collection practices for patients who are known to qualify for charity care or financial assistance. To be considered for charity care, however, the charity care policy states that the patient must cooperate with the Medical Center to provide the information and documentation necessary to apply for other existing financial resources that may be available to pay for health care services, such as Medicaid. Then the information is gathered and assessed in order to determine eligibility for charity care. Whenever possible, prior to admission, the Medical Center will conduct a pre-admission interview with the patient, the guarantor, and/or legal representative. Those patients who may qualify for financial assistance from a governmental program will be referred to the appropriate program, prior to the consideration for charity care. Part VI, Line 2: SWMC conducts a community needs assessment approximately every 3 years. The assessment includes conducting community focus group meetings to hear what the citizens believe are the community's most critical needs. Additionally, SWMC monitors Kansas Hospital Association patient origin and destination data to look for patterns of migration for services that might be made available locally as a new service. Part VI, Line 3: PFS refers self pay patients to Midland Professionals and Midland assists them in seeing if they are eligible for any federal, state, or local programs. The collectors send out charity care paperwork to patients as they are calling and collecting or if a patient calls for the packet. Part VI, Line 4: SWMC's total service area is rural and is made up of southwest Kansas and the neighboring areas of Oklahoma, Colorado, New Mexico and Texas. SWMC's primary service area, however, is Seward County, KS, which has a population of approximately 21,276. According to the U.S. Census Bureau this population is approximately 68.6% Hispanic, has a median household income of $62,269, and 14.4% of all people in Seward County are below the Federal poverty level.SWMC is a County owned hospital and is the only hospital in the county. It therefore provides care to all patients regardless of their ability to pay. 5.6% of services provided by SWMC are provided to uninsured patients while an additional 16.8% is provided to Medicaid patients. Part VI, Line 5: SWMC's seven Board of Trustees members are appointed by the Seward County Board of County Commissioners. Trustees are required to reside in Seward County, KS, and cannot be employed by the Hospital. SWMC extends medical staff privileges to all qualified physicians in the community who so desire to have such privileges.All surplus funds are reinvested back into the Hospital to fund capital needs and additional services that are identified as needed to continue to provide high quality services to the community. SWMC works with other community agencies, including the Seward County Health Department, to provide educational programs to the public throughout the year.
