This community service is specialized medical care for people living with a chronic disease. It provides relief from the symptoms and stress of a chronic disease-whatever the diagnosis-in your place of residence. Individuals do not need to be homebound to receive care. Palliative care does not replace treatment of an illness; the goal is to ease and prevent suffering and improve quality of life for both patient and family members. Along with symptom management, palliative care can facilitate discussions with patients and families concerning prognosis and goal planning. Palliative care may occur while you receive treatments for your illness. In 2017, Red River Palliative Care served 337 patients. 387079 0 8079 Bereavement & community support: the death of a loved one is one of the most stressful life events a person can experience, whether the death is expected or not. Hospice of the Red River Valley is committed to providing a robust bereavement program - not only to family members of the individuals we care for, but also for community members. Hospice of the Red River Valley's bereavement services are offered free of charge to anyone who has lost a loved one through death, whether they had hospice services or not. In 2017, Hospice of the Red River Valley's bereavement program served 1,593 new clients, facilitated 124 group sessions with 591 individuals attending, mailed out in excess of 7,711 bereavement support mailings, provided in-school grief support and presentations to more than 194 students and 20 faculty and developed new support groups specifically for men and infant and pregnancy loss. 38424 0 37633 Community Education and outreach: In 2017, we reached more than 4,400 individuals through presentations to churches, service clubs, fraternal organizations and community and health care organizations. More than 120 educational events were offered. In addition, Hospice of the Red River Valley's annual conference, The Journeying Home Conference, brought together more than 350 health care professionals for a day of hospice education. 6301 0 5135 Advance Care Planning: as a part of our commitment to community education, Hospice of the Red River Valley also provides specific training for healthcare professionals to prepare them to train their staff for having discussions with their patients regarding advance care planning and advance care directives. In 2017, Hospice of the Red River Valley trained 26 people to become First Steps advanced care planning facilitators. Additionally, we completed the Instructor Certification course for Last Steps Advance Care Planning and will be conducting these educational sessions for health care professionals as the 'next step' for facilitated discussions for those living with a life limiting disease. In addition to this education with health care professionals, Hospice of the Red River Valley has conducted several community education sessions throughout our region, educating the community on the importance of having discussions regarding advance care planning and to take action in completing their health care directives.
