ACCOUNTS ARE WRITTEN OFF WHEN THEY ARE DETERMINED TO BE UNCOLLECTIBLE BASED UPON MANAGEMENT'S ASSESSMENT OF INDIVIDUAL ACCOUNTS. IN EVALUATING THE COLLECTABILITY OF PATIENT ACCOUNTS RECEIVABLE, THE CORPORATION ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYOR SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR BAD DEBTS. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE CORPORATION ANALYZES CONTRACTUAL AMOUNTS DUE AND PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS AND A PROVISION FOR BAD DEBTS, IF NECESSARY. FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS, THE CORPORATION RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE BILLED RATES AND THE AMOUNTS ACTUALLY COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS CHARGED OFF AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS.THE CORPORATION'S ALLOWANCE FOR DOUBTFUL ACCOUNTS FOR SELF-PAY PATIENTS INCREASED TO 80% OF SELF-PAY ACCOUNTS RECEIVABLE AT JUNE 30, 2018, FROM 79% OF SELF-PAY ACCOUNTS RECEIVABLE AT JUNE 30, 2017. <ExplanationTxt>IN 2015 EVANGELICAL COMMUNITY HOSPITAL (ECH) COMPLETED ANOTHER COMMUNITY HEALTH NEEDS ASSESSMENT. THE ASSESSMENT WAS CONDUCTED BY TRIPP UMBACH ALONG WITH PARTNERING HOSPITALS AND OUTSIDE COMMUNITY ORGANIZATIONS AS A REGIONAL COLLABORATIVE EFFORT TO ASSESS THE NEEDS OF OUR COMMUNITY. EVANGELICAL'S NEEDS ASSESSMENT INCLUDED A FIVE COUNTY REGION CONSISTING OF SNYDER, UNION, NORTHUMBERLAND, LYCOMING AND JUNIATA COUNTIES. THE ASSESSMENT WAS COMPLETED IN APRIL OF 2015 AND ADOPTED BY THE EVANGELICAL BOARD OF DIRECTORS IN NOVEMBER 2015. FISCAL YEAR 2018 IS ECH'S SECOND YEAR OF REPORTING ON OUR IDENTIFIED NEEDS. DURING THIS ASSESSMENT, THE NEEDS OF OUR COMMUNITY WERE IDENTIFIED AS:1.) ADDRESSING NEEDS RELATED TO BEHAVIORAL HEALTH AND SUBSTANCE ABUSE; 2.) REDUCING THE IMPACT OF HEALTH CONCERNS RELATED TO LIFESTYLE; 3.) THE IMPACT OF SOCIO-ECONOMIC STATUS ON HEALTH OUTCOMES; AND 4.) INCREASING ACCESS TO HEALTHCARE. THIS PLAN PROVIDED A ROADMAP FOR OUR HOSPITAL TO MEET SPECIFIC NEEDS WITHIN OUR FIVE COUNTY SERVICE AREA. BELOW ARE THE FOUR KEY IDENTIFIED NEEDS, THE ACTION STEPS AND HOW THE HOSPITAL ADDRESSED THESE NEEDS FOR YEAR TWO OF THE 2015 IMPLEMENTATION PLAN:1. ADDRESSING NEEDS RELATED TO BEHAVIORAL HEALTH AND SUBSTANCE ABUSE:- THE CURRENT TELE-PSYCH SERVICE IS BEING UTILIZED APPROXIMATELY 15 TIMES PER QUARTER FOR THE ED AND APPROXIMATELY 6 TIMES A QUARTER FOR INPATIENT. RESPONSE TIMES ARE WITHIN THE EXPECTED RANGE (WITHIN 2 HOURS).- OPIOID REVERSAL KIT (NALOXONE) EDUCATION CONTINUES IN THE COMMUNITY WITH FAMILY/FRIENDS OF AT RISK PERSONS AS WELL AS FOR EMS, FIRE AND POLICE. SOME AREA BUSINESSES, SPECIFICALLY HOTEL STAFF HAVE RECEIVED EDUCATION. COMMUNITY HEALTH AND WELLNESS DEPARTMENT IS CURRENTLY RESPONSIBLE FOR THE DISTRIBUTION OF THE KITS AND IS WORKING ON AN EDUCATIONAL PROGRAM TO PROVIDE COMMUNITY AGENCIES AND BUSINESSES.- THE EMSO CONTINUES TO OFFER DEPRESSION SCREENING AND MAKE RECOMMENDATIONS REGARDING THE FINDINGS. WITH OUR CURRENT EHR THERE ARE LIMITATIONS TO TRACKING THE DATA THEY CAN EXTRACT RELATED SPECIFICALLY TO REFERRALS.- COMMUNITY HEALTH AND WELLNESS STAFF CONTINUE TO SUPPORT THE COMMUNITY THAT CARES (CTC) IN THE SHIKELLAMY SCHOOL DISTRICT. ONCE THE GRANT FUNDING HAS EXPIRED THE CTC WILL TRANSITION TO A UNITED WAY SUPPORTED PROGRAM. COMMUNITY HEALTH AND WELLNESS WILL CONTINUE TO SUPPORT EFFORTS. COMMUNITY HEALTH AND WELLNESS STAFF COULD NOT ATTEND THE "PROSPER" TRAINING DUE TO SCHEDULING CONFLICTS. POSSIBLE TRAINING OPPORTUNITIES MAY COME IN THE FUTURE.- HOSPITAL STAFF PARTICIPATED IN BOTH THE NORTHUMBERLAND COUNTY OPIOID COALITION AND THE SNYDER/UNION COUNTY OPIOID COALITION. CH&W STAFF WAS TRAINED TO EDUCATE FAMILY MEMBERS AND/OR FRIENDS OF THOSE ADDICTED TO OPIOIDS ON HOW TO ADMINISTER NALOXONE AND HOW TO RECOGNIZE SIGNS AND SYMPTOMS OF AN OVERDOSE.- ECH IS RESPONDING TO FOOD INSECURITY BY ASKING DIRECT FOOD AVAILABILITY QUESTIONS.2. REDUCING THE IMPACT OF HEALTH CONCERNS RELATED TO LIFESTYLE:- NEW WORKSITE WELLNESS CLIENTS HAVE INCREASED BY THREE.- AN ENDOCRINOLOGIST STARTED EMPLOYMENT SPRING OF 2018.- THE ORGANIZATION CONTINUES TO FOCUS ON INCREASING COLON SCREENINGS. WITH THE PURCHASE OF CENTRAL PENN GI, THE ABILITY TO IMPROVE THE VARIOUS SCREENING OPTIONS SHOULD OCCUR. THE CANCER COMMITTEE WILL BE USING COLON CANCER SCREENING TO ADDRESS STANDARD 4.2, "SCREENING PROGRAMS".- ADDITIONAL EDUCATION AND OUTREACH HAS BEEN DONE TO PRIMARY CARE OFFICES WITHIN THE SERVICE AREA REGARDING THE LOW DOSE CT LUNG SCREENING PROGRAM. TO DATE THIS FISCAL YEAR, 25 REFERRALS WERE RECEIVED, COMPARED TO 8 LAST FISCAL YEAR TO DATE.3. INCREASING ACCESS TO HEALTHCARE:- AT THIS TIME, ECH IS NOT WORKING ON ANY NEW INITIATIVES WITH THE PLAIN COMMUNITY. PROGRAMMING THROUGH THE USE OF THE MOBILE MEDICAL UNIT WILL TARGET AREAS WHERE THIS POPULATION LIVES. PRIMARY CARE AND COMMUNITY OUTREACH PROGRAMS HAVE BEEN SCHEDULED FOR THESE SPECIFIC AREAS.- THE PLAIN COMMUNITY DISCOUNT WAS RE-EVALUATED AND PRICING HAS BEEN ADJUSTED AS OF 6/1/2018 TO BE COMPETITIVE WITH OTHER INSTITUTIONS OFFERING DISCOUNTS AS WELL. THE PLAIN COMMUNITY PROGRAM HAS SEEN A 6.4% INCREASE IN UTILIZATION OF THE PROGRAM AND A 24.1% INCREASE IN GROSS REVENUE FROM LAST FISCAL YEAR TO THIS FISCAL YEAR TO DATE.- FY18 UTILIZATION OF PLAIN COMMUNITY PROGRAM:ECH ENCOUNTERS CHARGESFY2017: 2,266 $6,855,403FY2018: 2,268 $8,309,429EMSO ENCOUNTERS CHARGESFY2017: 2,221 $799,977FY2018: 2,662 $1,351,370- AREAS IDENTIFIED TO TARGET WITH MOBILE HEALTH OF EVANGELICAL ARE SPRING MILLS, LOGANTON, MCALISTERVILLE, MONTGOMERY, WASHINGTONVILLE, RUSH TOWNSHIP, SHAMOKIN, SUNBURY, BEAVER SPRINGS, PORT TREVORTON, LAURELTON, AND MILLMONT.- MOBILE HEALTH OF EVANGELICAL WAS ESTABLISHED AND PRIMARY CARE, CARDIAC AND WOMEN'S HEALTH VISITS HAVE BEEN MADE TO A VARIETY OF LOCATIONS THAT WERE IDENTIFIED AS A PRIORITY FOR THE FIRST YEAR OF SERVICE IN THE PRO FORMA.4. THE IMPACT OF SOCIO-ECONOMIC STATUS ON HEALTH OUTCOMES:-- THE "ASK ME 3" PROGRAM WAS RE-DESIGNED AND PROVIDED TO AREA COMMUNITY GROUPS.
