This group of organizations was selected due to their involvement with the broader community as well as involvement with unique populations. In total 22 individuals from these organizations were interviewed. St. Joseph Hospital Part V, Section B, Line 11: The prioritization of community health needs began by correlating data received from primary data sources (community survey, medical community survey and key informant focus interviews). This data was analyzed to ascertain the most prevalent community health needs noted from each constituent group. Secondary data was then utilized to formulate a more comprehensive evaluation of the health need. Once the identified health needs had sufficient secondary data, the information was evaluated to determine common themes among the three constituent groups.Through this process five significant community health needs were identified. These needs were then prioritized utilizing a point system relative to four components: severity of health need (utilizing primary and secondary data sources), estimated feasibility and effectiveness of possible interventions, the level of importance the community, medical community and key informant focus interviews placed on the need (determined by listing within top three barriers to health or listing within top three most pressing health concerns), and synergy between two or more constituent groups relative to importance of health need. The implementation plan that was developed identifies a number of actions that St. Joseph Hospital will take to address these health needs. In areas where the Hospital does not have the required expertise or resources to address these needs, the implementation plan identifies actions that will provide support to other community organizations that are better able to address these needs.Recognizing that there will be limited funds available to address the full list of community health needs, and recognizing that other agencies and organizations in the area had expertise or missions in these areas, there are a number of health needs that were identified by some respondents that are not included in the implementation strategy. These include: cancer, dental problems, diabetes, domestic violence, infectious diseases, suicide, and teenage pregnancy. Schedule H, Part V, Section B, Line 8: A second Community Health Needs Assessment was conducted in 2016 and an Implementation Strategy was adopted in the same year. There were six areas of priority identified, such as: substance and alcohol use disorder, mental health, obesity, cancers, aging problems and diabetes. St. Joseph Healthcare then developed a robust implementation strategy to address four of these needs. In 2017 St. Joseph Healthcare addressed the substance and alcohol use disorder with strategic initiatives including awareness building in the community, implementing prescribing protocols in our primary care facilities, training our primary care providers in substance use disorder treatment, providing enhanced access to Subaxone treatment and by improving access to the rapid response kits in partnership with Community Health Leadership Board, which is a collaboration of nine major healthcare agencies in the area. We will continue to address the issue of obesity adding an additional "Healthy Lifestyle Program" available to patients in need while continuing the educational and awareness building presence in the social media though "Healthy Active Living." We will also partner with other local agencies and programs to provide food security resources to patients and families who are in need.In 2017, we partnered with the Eastern Area Agency on Aging in educating and providing awareness to the public regarding resources available for the aging population. St. Joseph Healthcare conducted two workshops including the screening of "Being Mortal" to facilitate the end of life care decision making and providing resources for the same.In 2017 St. Joseph Healthcare provided training to our employees in caring for patients with behavioral disorders and will continue to provide clinical expertise in the hospital focusing on the inpatient population.Despite these new initiatives, St. Joseph Healthcare is committed to providing counselling services to facilitate enrollment of community members within the Affordable Care Act marketplace insurance exchange through certified enrollment support throughout the community. St. Joseph Healthcare also collaborates with area agencies to address food insecurity in this community and provides food and grocery items for anyone in need. Employees provided hours of service to start and maintain the St. Joseph Closet, St. Joseph Food Pantry and St. Joseph Cupboard which benefit patients, families, employees and the community. Provided community benefit through several awareness building and educational programs as part of public awareness building on human trafficking. Supported the Vegetable Exchange Stand to provide low-income individuals with the opportunity to access fresh fruits and vegetables. Excess produce from home gardeners is donated to the stand and community members are invited to enjoy this produce at no charge. Partnered with Good Shepherd Food Bank to provide 355 individuals with bags of nonperishable food for three days and resources for local food sources.Served an evening meal once per month to the Greater Bangor Area Homeless Shelter for a total of 259 meals. Dietary provided food assistance to address food insecurity for patients. Established a supportive partnership with Kingman Elementary School to ensure food security, enhanced educational opportunities, improved educational tools and connectivity relative to health and wellness education. Nutrition Services provides complimentary food and beverages to family members who sit vigil with loved ones who are nearing the end of life. Nutrition Services also provided 1,080 healthy meals for the residents of Hope House. During 2018, St. Joseph Healthcare continued to show its commitment to the community through its initiatives, leadership, cash and in-kind donations. Part VI, Line 2: In 1947, when the Felician Sisters bought the Paine Private Hospital, renamed it St. Joseph Healthcare and began their ministry in this community, it was a long-time commitment they were offering the residents of the greater Bangor area. This commitment was not only to care for the sick in this community, but also to keep the community healthy and provide care for people who are poor and vulnerable. When the ownership of this faith-based community hospital was transferred to Covenant Health, this commitment was renewed with even more passion and striving for excellence. Covenant Health's 4 values (compassion, integrity, collaboration and excellence) are reflected through our daily business, which has made this organization a beacon of hope and a place of healing and compassion.Honoring the legacy of this organization, St. Joseph Healthcare continues to care for the poor and vulnerable in our society. We have been proactive and creative in providing appropriate assistance where needed. Currently, this commitment to caring for the whole person manifests itself through community outreach programs, activities and services designed to support community members' health and wellness needs. It also supports the growth and sustainability of a healthy and productive community. By partnering with and supporting agencies with a similar mission and goals, St. Joseph Healthcare is able to provide persons and communities with a higher level of service, while also addressing some of the social issues facing us today. Part VI, Line 3: Each inpatient receives a packet upon admission that includes a description of the Hospital's Benevolence (Free Care and Cost Share) Policy. If an inpatient has any questions during their stay, a patient ombudsman is available to assist. Signage is placed in all outpatient registration sites and applications are available. Staff is available to answer questions. Upon billing, included with the statement is a notification that free care and cost share is available, the poverty guidelines, and a telephone number for assistance. The Policy is also posted on the Hospital's website. Part VI, Line 4: The Hospital's primary service area is Penobscot County which comprises the county seat and the third most populated city in the State of Maine: Bangor. The County consists of a total area of 3,556.14 square miles. Within the Hospital's primary service area of Penobscot County the southern and western regions of the County account for more than 50% of the inpatient admissions. The Hospital's secondary service area includes the Penobscot County border communities such as the towns of Bucksport and Ellsworth (located in Hancock County), Milo and Dover-Foxcroft (located in Piscataquis County) and Winterport (located in Waldo County). The U.S. Census report estimated as of July 1, 2018 that the population for Penobscot County as 151,096; recognizes that 94.8% are white, 50.5% are female; 18.7% are 65 years of age or older; 14.3% are below the poverty level; and the median household income is $47,886. One way in which St. Joseph Healthcare strives to support these activities is through the education of the next generation of health care leaders who will be called to continue the Catholic health care ministry. St. Joseph Healthcare assisted aspiring physician assistants, students of nursing, including nurse practioners, bachelor level nurse degree programs, and nursing partnerships. St. Joseph Healthcare also provided health professional educational opportunities for individuals within the following health profession areas of study: emergency medical services, social work, health information management, medical technologist, medical assistant, pharmacy, physical and respiratory therapy, radiography, and administration. St. Joseph Healthcare recognizes the importance of enhanced training and educational opportunities for members of Emergency Medical Services organizations, including those within rural and volunteer EMS associations.St. Joseph Healthcare has provided community health improvement services and outreach to many within the greater Bangor community. The Community Health Needs Assessment identified a need to educate the public on health and wellness opportunities. St. Joseph Healthcare created an extensive media presence with "Choosing Wisely" program. This program provided educational materials relative to weight loss, healthy living, exercise, etc. The program included TV programs, heavy Social Media presence and printed literature all through the health system. Several initiatives were undertaken to support community health and wellness; these initiatives specifically focused on education of providers, health leaders, and community members. Initiatives focused around food security, human trafficking, sexual assault, and community health and wellness outreach. The Spiritual Care Department actively provides spiritual support to patients and their family members. The Spiritual Care Department support extends to any member of the community who seeks assistance. In addition, the Spiritual Care Department implemented a 12-month bereavement outreach program in which family members are provided consistent bereavement support materials and opportunities for spiritual support through the grieving process. Nurses provided healthcare screening to our homeless population through blood pressure clinics benefitting 96 persons. St. Joseph Healthcare provided support to a pulmonary support group, the Better Breathers' Club, to support individuals with chronic lung disease to enhance their lifestyle through education and community support. Leadership provided education and community services through the Maine Multi Cultural Centre. St. Joseph Healthcare provided community services through the ACI Physician Practices to support Courage Lives (formerly known as Hope Rising), a residential recovery program for victims of human trafficking. St. Joseph Healthcare provided health and wellness educational programming for seniors, Senior Wise Meal/Education Program-providing important health information and discounted pricing for nutritious meals. Part VI, Line 6: St. Joseph Healthcare is comprised of St. Joseph Hospital (an acute care hospital), St. Joseph Ambulatory Care, Inc., (Infusion Therapy services and employer health services), and Alternative Health Services, Inc. (which provides home health and hospice services). By joining Covenant Health, St. Joseph Healthcare is aligned with two other acute care hospitals and multiple skilled nursing and assisted living facilities. Part VI, Line 7, Reports Filed With States ME Part VI, Line 5 (continued): St. Joseph Healthcare supported community benefit operations through a number of activities including several public education campaigns and programming, a community vegetable exchange stand and programming focused on the elder population. The elder population was an area of focus for outreach and educational activities in collaboration with the Senior Center. Employees arranged speakers and educational programming. St. Joseph Healthcare coordinated a community fruit and vegetable stand during the summer months in which local gardeners donated their excess produce to the stand and community members were welcome to take these items free of charge. The stand was quite popular with many individuals expressing that they did not have the means to purchase fresh vegetables and that this was their only opportunity to enjoy these items.
