Hawkins Memorial Fund was established to benefit DCH Regional Medical Center employees with priority given to the Obstetric-Nursery group. The Benjamin Louis Ladner Memorial Scholarship Fund was established to improve and increase the quality and quantity of healthcare to patients in west Alabama by awarding scholarship assistance to individuals pursuing a higher education degree in nursing. The Pettus Randall Pastoral Care Program was established to help meet the mission of the pastoral care program with the Cancer Treatment Center at DCH. Part X, Line 2: The Foundation is exempt from federal income taxes under the provisions of the Internal Revenue Code Section 501(c)(3) and also is exempt from state income taxes under the applicable income tax regulations of the State of Alabama. The Foundation is exempt from taxes on income other than unrelated business income, if any. In addition, the Foundation is not considered to be a private foundation. The Foundation files income tax returns in the U.S. federal jurisdiction. The Foundation has determined that it does not have any material unrecognized tax positions as of September 30, 2018, and there are no interests and penalties related to income tax assessments. It makes donations to DCH to support various programs and awards scholarships to college students. Form 990, Part VI, Section B, line 11b Foundation personel as well as board members review the completed form 990 to determine if any changes or corrections need to be made prior to filing. Form 990, Part VI, Section B, line 12c Prior to the Foundation entering into a transaction or arrangement, board members are required to disclose any potential conflicts of interest. After the individual discusses all the facts and answers any questions, he or she then leaves the committee or board meeting while the remaining members discuss and vote on whether a conflict of interests exists.
