The focus is grant-making to programs and initiatives that improve the health of underserved populations in the Twin Cities and throughout Minnesota and western Wisconsin. 0 0 395170 390000 0 In 2018, UCare Foundation distributed four community grants to Minnesota organizations working to improve the overall health of people of all ages, cultures, and abilities through innovative services, education, community outreach and research. UCare Foundation grants were awarded to organizations meeting one of four health improvement-related criteria for 2018:-Preventive health care-Quality initiatives for chronic disease management-Programs for people with disabilities-Promoting health equityIn 2018, grants provided by the Foundation using funds contributed by UCare included support for a newspaper that promotes social inclusion and legal rights of people with disabilities; a mobile grocery store that increases access to healthy, affordable food in under-resourced area, a program to promote healthy lifestyles and habits in the African American community, and programs that provide various supports to aging seniors who are struggling to live independently in the community and experiencing social isolation. UCare Foundation has a formal board that oversees and approves annual grant awards. The board meets annually to determine priority areas of focus for current year solicitations. Grantees apply via an online grant system and all organizations must meet funding eligibility guidelines. Grant applications are reviewed by UCare Minnesota-assigned individuals as well as certain members of the UCare Foundation board. Once approved, grantees sign a conditions of grant agreement and are responsible to submit an interim and final report as well as participate in a site visit by UCare staff.
