The CFSC Grants Committee reviews the reports and deals with any questions. The grant recipents in 2020 are all organizations known for their responsibility accountability. Form 990, Part VI, Section B, Line 11b Each member of the Board of Directors of the Community Foundation of San Carlos was provided with a complete copy of the completed Form 990 before it was filed. They were asked to bring forward any questions they might have and to provide any feedback they wished. Form 990, Part VI, Section B, Line 12c The CFSC Conflict of Interest Policy states, The foundation requires all board members to disclose any potential conflicts and sign an acknowledgement that they have read, understood and agreed to follow policy and that their disclosures are complete and accurate to the best of their knowledge. Disclosure of possible conflicts should be updated as new possible conflicts arise. In addition, the CFSC Bylaws provide the following in Section 10.3 The Board shall periodically review, no less than annually, any director, officer, agent or employee conflicts of interest and shall require an annual certification from each of them disclosing conflicts of interest. In Section 10.4 If the Board of Directors determines that an interested person has failed to disclose an actual or potential conflict of interest, it shall take appropriate disciplinary and corrective action, as the Board may determine at its sole discretion. Form 990, Part VI, Section C, Line 19 There have been no specific requests during the year for inspection of CFSCs governing documents, conflict of interest policy or financial statements. If requested, these documents would have been willingly shared. However, when CFSC went before the San Carlos City Council to present the details of its formation, Form 1023, the Bylaws, the Investment Policy, and the Grants Policy were a part of the Councils Board Packets for the meeting. They were also available to community members by request to the City. Schedule B, Part I, Section Column b, Line Lines 2, 3, 4, 5 Donations from these individuals came from their Donor-advised Funds at major financial institutions. The name of the donor of the fund is listed with a DAF designation after the name. IRS has not yet given specific guidance on how to handle donations of this sort on Schedule B.
